Page 2 of 2 SOURCES SOUGHT NOTICE This is a Request for Information (RFI), Sources Sought issued for market research and planning purposes only. This is not a Request for Quotes (RFQ) or Request for Proposals (RFP). No formal solicitation document exists at this time. The Department of Veterans Affairs (VA), Veterans Integrated Service Network (VISN) 04 has a need for orthotic appliances, braces, custom foot orthotics, orthopedic shoes, orthopedic shoe modifications, and related services to eligible Veterans. The purpose of this Sources Sought Notice is to identify interested vendors with the resources and capabilities to provide veteran patients the convenience of same-day service with the proper fitting and fulfillment of orthopedic shoes and inserts (both standard and custom) for the following VA Medical Centers: Corporal Michael J Crescenz VA Medical Center, 3900 Woodland Avenue, Philadelphia, PA 19104 Coatesville VA Medical Center, 1400 Blackhorse Hill Road, Coatesville, PA 19320 Wilmington VA Medical Center, 1601 Kirkwood Highway, Wilmington, DE 19805 Wilkes-Barre VA Medical Center, 1111 East End Blvd., Wilkes-Barre, PA 18711 Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 The NAICS code is 339113, Surgical Appliance and Supplies Manufacturing, Size Standard is 750 employees and the requirement is planned for a five (5) year period of performance. Scope of Work: The Contractor shall be required to attend all weekly VA orthotic appliance and shoe clinics or VA Community Living Center (CLC) appointments with a certified staff Orthotist for orthotic appliances and Pedorthist for custom foot orthotics, orthopedic shoes, and orthopedic shoe modifications in support of the evaluation of the patient, casting, fabrication, and fitting. Prospective and participating vendors must be able to properly fit and have on hand and/or easily available stock inventory of orthopedic shoes and inserts (the most common men and women sizes in compliance with the current national mandatory contracts-MSPV/FSS) and instruct patients on the proper wearing and care of issued items in a same day service capacity (specialty and custom item exclusions apply). The Contractor shall provide all orthotic appliances, custom foot orthotics, orthopedic shoes, and orthopedic shoe modifications in strict conformance to the prescription that has been provided by the VA requesting provider and conformance with any national administrative and contract authorities. The Contractor shall not add or substitute components or alter the Veteran s prescription in any way without prior approval and written authorization from the VA Clinic Team, Prosthetic Representative or designee. Products that are not commercial items are considered experimental and purchase of experimental products shall not be authorized under this solicitation. The overarching goal of this acquisition is to establish multiple contracts or a Blanket Purchase Agreement to provide orthotic appliances, custom foot orthotics, orthopedic shoes, and orthopedic shoe modifications and related services to eligible Veterans at the Coatesville, Lebanon, Philadelphia, Wilkes-Barre and Wilmington VA Medical Centers. This Sources Sought notice is issued in an attempt to determine level of interest among both Large and Small businesses including qualified VETBIZ registered Service Disabled and Veteran Owned Small Businesses (SD/VOSB). Responses will assist VA planners in determining an appropriate acquisition and/or set-aside strategy which best fits VA requirements. Interested vendors are requested to respond to this notice no later than 3:00 PM EST on September 7, 2018, via email to Stanley.Kwasny@va.gov. Vendor response shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email of primary point of contact (e) DUNS number (f) Type of business (e.g. Large or Small). - If vendor is a Small business, provide type of small business. If applicable, (e.g. Services Disabled Veteran Owned Small Business(SDVOSB), Veteran-owned small business (VOSB), 8(a), HUB-Zone, Women Owned Small Business, Small Disadvantaged business, or Small Business). - Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) vendors shall provide proof of registration and verification in Vendor Information Pages (VIP) database at www.VetBiz.gov. (g) A capability statement: MAXIMUM two (2) pages (additional attachments/pages will not be reviewed) summarizing experience and past performance in providing orthotic appliances, custom foot orthotics, orthopedic shoes, orthopedic shoe modifications, and related services. (h) If currently a vendor under the VA Federal Supply Schedule (FSS) or other government contract please provide contract number(s) and listing of Special Item Number(s) (SIN) covered. If not a vendor with a current FSS contract please so state in the capability statement. (i) Past Performance experience: Respondents are also encouraged to provide specific examples (e.g. contract number, point of contact information) of the vendor s experience providing the same or similar services to that described in this Sources Sought.
