THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited. The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following service(s) according to the statement of work below for the Fayetteville VAMC, 2300 Ramsey Street, Fayetteville, NC 28301 from 10/1/2018 - 9/30/2019: Item No. Schedules of Supplies/Services Estimated Quantity Unit 0001 Milk, whole, fresh, pasteurized, homogenized Vitamin A and D fortified, ½ pint container, Type 1 Class 1 15,500 EA 0002 Milk, low fat (2% milk fat) fresh, homogenized, pasteurized, vitamin A and D fortified, 1/2 pint container 42,000 EA 0003 Milk, skim, nonfat (less than 0.5% milk fat), fresh, milk solids added, homogenized and pasteurized, vitamin A and D fortified, ½ pint container 15,000 EA 0004 Milk, chocolate flavor, 1% 2% milk fat, fresh, homogenized, pasteurized, vitamin A and D fortified, ½ pint container 10,000 EA 0005 Vanilla Ice Cream, regular, 4 oz container, 2 dozen per package 200 PK 0006 Chocolate Ice Cream, regular, 4 oz container, 2 dozen per package 120 PK 0007 Strawberry Ice Cream, regular, 4 oz container, 2 dozen per package 120 PK 0008 Orange Sherbet, Regular, 4 oz container, 2 doz per package 140 PK DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK: All orders shall be received at the Nutrition and Food Services Building (Building 3), 2nd Floor, Room 209 at the VA medical Center, Fayetteville, NC within the specified time-frames: Deliveries to be made not before 6:00am and not after 2:00pm, Monday through Friday. Deliveries shall not be made on Saturday, Sunday or Federal Holidays, except in extreme emergencies. Deliveries shall be made two (2) times per week and not on consecutive days; and at least once (1) per week for ice cream and sherbet products. The VA needs enough time before the expiration date on product to allow reasonable rotation. Milk and milk product delivery shall occur 10+ days before the expiration date on the delivered product package, and milk delivery temperature must be at 38 degrees or less and ice cream/sherbet must be delivered in a frozen state. Payment will be made monthly upon receipt of properly prepared invoices for items delivered and accepted during the invoice period. The proper purchase order number shall be stated on each invoice. Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors will be required to be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award. Your response is required by 1:00 PM on Friday, September 7, 2018.
