Page 1 of 5 COMBINED SYNOPSIS/SOLICITATION VANJHCS MUSICIAN FOR WORSHIP SERVICES AT THE NEW JERSEY HEALTHCARE SYSTEM (NJHS), EAST ORANGE CAMPUS (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items and FAR part 13.5- Simplified Procedures for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written pre-solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number 36C24218U0042. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. (iv) This procurement is being issued as a Notice of Small Business Set-Aside and applies to all items in this solicitation. The North American Industry Classification System (NAICS) code is 711130 Musical Groups and Artists, with a small business size standard of $11 Million. (v) Contractor shall complete attached Price/Cost schedule as a qualifying response. (vi) Description of requirement (See attached Performance Work Statement). The contractor shall provide all parts and labor identified in the Performance Work Statement. (vii) Awardee shall coordinate with the Contracting Officer s Representative (COR) prior to performance. Place of Performance: New Jersey Healthcare System (NJHS), East Orange Campus 385 Tremont Avenue East Orange, New Jersey 07018-1023 (viii) The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The following provisions are included as addenda to FAR 52.212-1: 52.252-1 Solicitation Provisions Incorporated by Reference (FEB 1998) 852.252-70 Solicitation Provisions or Clauses Incorporated by Reference (JAN 2008) 852.270-1 Representatives of Contracting Officers (JAN 2008) (ix) The provision at FAR 52.212-2, Evaluation Commercial Items (OCTOBER 2014), applied to this acquisition. Award shall be made to the quoter whose quotation offers the best value to the government, considering technical capability, past performance, and price. The government shall evaluate information based on the following evaluation criteria: (1) Technical Capability: Provide a technical capability statement demonstrating how all requirements of the scope of work will be met. This must address past experience in conducting Musician for Worship Services. Please provide a maximum 5-page capability statement; (2) Past performance, please provide three references for contract of similar scope and magnitude to be evaluated, the contractors Past Performance shall also be reviewed in CPARs. Contractors with no past performance will be rated as neutral. (3) Price, the government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Evaluation of options will not obligate the Government to exercise the option(s). The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable. (xiv) N/A (xv) This is an open-market total small business set-aside combined synopsis/solicitation for VANJHCS Musician for Worship Services at New Jersey Healthcare System (NJHS), East Orange Campus.  The government intends to award a Firm Fixed Price (FFP) contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" The Quoter shall list exception(s) and rationale for the exception(s). Quotes received without this statement will be assumed to infer that terms and conditions are acceptable without modification, deletion or addition. Submission shall be received not later than Tuesday, September 11, 2018, 10:00 AM EST. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). Submittal of all questions will not be accepted after Wednesday, September 05, 2018 @ 10:00am EST. VANJHCS Musician for Worship Services at the VA New Jersey Healthcare System The Musician for worship services at the VANJHCS in East Orange is under the direct supervision of the chaplain presiding on the particular event(s). The Musician performs supportive music for the particular congregation. In addition, the Musician shall be prepared to provide music which will establish a meditative space prior to the service as congregants arrive. When appropriate, the Musician works directly with Veterans attending soliciting special requests. If using VA owned instruments, the Musician will report any deviation in the quality or performance of the piano, organ or other equipment used. The Musician reports directly to the designated worship service area, at 8:30am whether it be the Auditorium, Day Room or Chapel. When performing more than one service which is every Sunday, the Musician shall be accompanied by the presiding chaplain. Total time required is approximately one hour per service and a total of two services per Sunday. The musician performs from 9am to10am on the third floor in the Chapel, Auditorium or Day Room than again at 10 am to 11am on the unit at East Orange Campus. The Musician shall have no access to patient records. The musician will have no access to cyber equipment. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The musician is a protestant who will provide musician services at our East Orange Campus. These services are performed on two separate units on the campus for our Veterans. The performances will take place every Sunday. Background: Worship services for inpatient Veterans is guided by VHA Handbook 1111.2. that states that compensation for musical worship service be provided. This Constitutional mandate provides for the expression of religion. The VA accomplishes this by providing worship services for its major faith groups. Musicians support the culture of the worship service for Protestants to more closely approximate the religious experience available when not hospitalized. A needs assessment determines the number and location of services required. This task is not currently technology related. Period of Performance: The contractor shall report to designated worship services on the morning of the worship service on Sundays at least 15 minutes before the start time. The performance is for the entire fiscal year of 2019-2023 from October 1, 2018 thru September 30, 2023 every Sunday from 9 o clock am until 11o clock am.
