- Home Oxygen Requirement: The VA Medical Centers provide Home Oxygen delivery and set-up to Veteran beneficiaries in Pennsylvania. Geographical areas covered under this requirement (Counties serviced but not limited to) for Altoona VAMC: Bedford, Blair, Cambria, Cameron, Centre, Clearfield, Clinton, Elk, Huntingdon, Indiana, Jefferson, Juniata, Mifflin, and Somerset Counties, West Moreland, Allegheny; Butler VAMC: Pennsylvania Counties- Allegheny, Armstrong, Beaver, Butler, Clarion, Forest, Lawrence, Mercer, Venango; Ohio Counties- Ashtabula; West Virginia Counties- Marshall; Erie VAMC: New York Counties- Chautauqua, Cattaraugus; Pennsylvania Counties- Warren, Venango, Crawford, Mercer, Forrest, Cameron, Potter, Jefferson, Kane; Ohio Counties- Ashtabula, Lake, Geauga, Trumbal; Pittsburgh VAMC: Ohio counties- Belmont, Columbiana, Harrison, Jefferson, Mahoning, Portage, Trumbull, Muskingum; Pennsylvania Counties- Allegheny, Armstrong, Beaver, Butler, Clarion, Fayette, Greene, Indiana, Lawrence, Washington, Westmoreland, Venango; West Virginia Counties- Hancock, Marshall, Ohio, Tyler; Maryland Counties- Garrett. The Contractor shall arrange for Home Oxygen for veterans traveling and/or temporarily relocating to another catchment area. The Contractor shall be responsible for providing Home Oxygen supply to Veteran beneficiaries serviced by VA Medical Center Prosthetics and Sensory Aid Service (P&SAS). Due to the monthly fluctuation of patients, task orders will be issued on a monthly basis. The contractor shall provide all supplies, materials, equipment, transportation of equipment, set-up, labor, supervision, patient education, safety management, and infection control, as necessary, for patients on home respiratory care therapy, as specified herein and authorized by the Contracting Officer Representative (COR). These services shall be required 24 hours a day, seven (7) days a week, including holidays. The contractor shall be staffed and stocked to render Home Oxygen delivery and set-up provided in accordance with all contract terms and conditions for the duration of the performance period. It is the Offerors/Contractors responsibility to thoroughly review the specifications and become familiar with the area of coverage in order to be fully aware of the Home Oxygen delivery and set-up requirements.
