This procurement is for new OEM medical equipment only for VA Medical Centers. No remanufactures or gray market items will be acceptable.   (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. ATTACHMENT 2 STATEMENT OF REQUIRMENTS Statement of Requirements Replacement Conference Room furniture Harry S Truman Memorial Veterans Hospital request to replace 82 Executive Conference Room Chairs, and 60 side/guest chairs in the conference rooms located within the facility. Conference rooms 553A, 546, Pro Dental Building Conference Room, G017, and HR Building Conference Room. This request is required due to many of the chairs currently in place are old and have met their end of life expectancy. Safety of the employees is to be considered. SCOPE OF WORK This performance work statement describes the requirement for procuring, set up, delivery, and installation of 82 executive conference room chairs and 60 guest/side chairs. All work shall be performed on the fifth floor of the Harry S Truman Memorial Veterans Hospital. Work to take place Monday thru Friday 8:00 am-4:30 pm Freight elevators are available and within reasonable amount of access to transport chairs to the designated room. Assemble chairs on site. Deliver to designated rooms. Conference Rooms 553A, 546, Pro Dental Building Conference Room, G017, and HR Building Conference Room. Removal/disposal of old chairs. Remove old chairs from facility and discard. Removal of all debris, cardboard and trash will be the responsibility of the installation team. Truman VA does not have storage space to store chairs. No direct shipment will be accepted. Comply with emission guidelines set by Greengard Environmental Institute REMOVAL/DISPOSAL OF OLD CHAIRS Removal/disposal of all old chairs DELIVERY AND INSTALLATION Delivery, Set-up, and Installation shall be coordinated with Patricia Beasley at 573-814-6661 or patricia.beasley@va.gov. Vendor shall review all items needed for this project with the COR Interior Designer listed above to ensure all items requested on the project are included and final placement is correct. Vendor shall install furniture level and true according to manufacturer's written instructions to assure proper operation. The vendor must clean furniture surfaces after installation, according to manufacturer's written instructions. Vendor will remove all old chairs from the facility and discard. The vendor must engage Installer to train Owner's maintenance personnel to adjust, operate and maintain all furniture and seating products. The installation shall be only by manufacturer qualified and certified installers with the knowledge of commercial furniture installations and shall comply with all necessary VA and safety regulations. SITE CONDITIONS The awarded Vendor shall include removal of all related shipping debris and construction cleanup associated with delivery and installation of the specified items. All packaging must be taken off the Truman VA premises by the Vendor. The Vendor shall be responsible for any damage to the building that occurs due to Vendor error or neglect. The Vendor shall be responsible for the coordination of staging areas and parking within the receiving area of the building. All Furniture deliveries, installations, and arrangements shall be coordinated with the COR Interior Designer. There is a loading dock at this facility. Service elevator is available. Detailed information of the elevator is available if needed. The contractor is expected to protect the interior of the elevator to prevent damage to the interior walls of the elevator.
