Page 2 of 2 SOURCES SOUGHT NOTICE This is a Request for Information (RFI) Sources Sought issued for market research and planning purposes only. This is not a Request for Quotes (RFQ) or Request for Proposals (RFP). No formal solicitation document exists at this time. The Department of Veterans Affairs, Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 has a need for programmatic and clinical consultation, training and evaluation services for existing Mental Health Intensive Case Management (MHICM) program staff at the Lebanon VA Medical Center for the development and implementation of Integrated Dual Diagnosis Treatment (IDDT). The IDDT model combines substance abuse services with mental health services to help treat veterans with severe mental health illness and substance abuse disorders concurrently in the same service organization by the same team of treatment providers located at the Lebanon VA Medical Center. The addition of services will allow our program to treat a cohort of Veterans with serious mental illness (SMI) and substance use disorders (SUD) in a community based intervention model. These specific care needs are not currently being met through existing services. The overarching goal of this acquisition is to acquire programmatic and clinical consultation and training for existing the Mental Health Intensive Case Management (MHICM) program staff at the Lebanon VAMC in order to enable treatment of veterans with both severe mental health illness and substance abuse disorders concurrently in the same service organization - by the Mental Health Intensive Case Management (MHICM) program team of treatment providers located Lebanon VAMC. This Sources Sought notice is issued in an attempt to determine level of interest among Large and Small businesses including qualified VETBIZ registered Service Disabled and Veteran Owned Small Businesses (SD/VOSB). Responses will assist VA planners in determining an appropriate acquisition and/or set-aside strategy which best fits VA requirements. The NAICS code is 611710 and the size standard is $15 million. Interested vendors are requested to respond to this notice no later than August 31, 2018, 3:00 PM EST via email to Stanley.Kwasny@va.gov. Vendor response shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email of primary point of contact (e) DUNS number (f) Type of business (e.g. Large or Small). - If vendor is a Small business provide type of small business. If applicable, (e.g. Services Disabled Veteran Owned Small Business(SDVOSB), Veteran-owned small business (VOSB), 8(a), HUB-Zone, Women Owned Small Business, Small Disadvantaged business, or Small Business). - Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) vendors shall provide proof of registration and verification in Vendor Information Pages (VIP) database at www.VetBiz.gov. (g) A capability statement: MAXIMUM two (2) pages (additional attachments/pages will not be reviewed) summarizing experience and past performance in providing programmatic and clinical consultation and training services for the development and implementation of Integrated Dual Diagnosis Treatment (IDDT). (h) If currently a vendor under the VA Federal Supply Schedule (FSS) please provide contract number(s) and listing of Special Item Number(s) (SIN) covered. If not a vendor with a current FSS contract please so state in the capability statement. (i) Past Performance experience: Respondents are also encouraged to provide specific examples (e.g. contract number, point of contact information) of the vendor s experience providing the same or similar services to that described in this Sources Sought.
