This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ) and the solicitation number is 36C24418Q9466. The solicitation document and incorporated provisions and clauses are those in Page 3 of 21 effects through Federal Acquisition Circular 2005-96 effective November 06, 2017. The Department of Veterans Affairs, Altoona VAMC 2907 Pleasant Valley Blvd, Altoona, PA 16602 is seeking to purchase Boiler Plant Inspection and Testing Services. All interested companies shall provide quotation(s) on the attached line items. The anticipated award will be a 5-year firm fixed price zero dollar BPA. Period of Performance will be established for yearly anticipated orders in each line item. Contract Line Item Numbers: CLIN 0001: Service Contract for yearly inspection and testing (in September) per attached SOW and the VHA Boiler Plant Safety Device Testing Manual Fourth Edition). ***Base Year service will be in September*** (1 JB). CLIN 1001: Service Contract for yearly inspection and testing (first week in August) per attached SOW and the VHA Boiler Plant Safety Device Testing Manual Fourth Edition). (1 JB) CLIN 2001: Service Contract for yearly inspection and testing (first week in August) per attached SOW and the VHA Boiler Plant Safety Device Testing Manual Fourth Edition). The government will evaluate information based on the following evaluation criteria: Price All work shall be completed in accordance with the Statement of Work (SOW). The SOW is included as Attachment 1 to this solicitation. Offerors are requested to quote Firm Fixed Price for any and all of the line items on the Schedule of Supplies and Services attached to this solicitation. Service Contract Act Wage Determination Number WD15-4221, Revision Number -8, dated 01-16-2018, applies to this acquisition and will be incorporated into the resulting contract. The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items FAR 52.212-3, Offerors Representations and Certifications Commercial Items Offerors must complete annual representations and certifications on-line at http://orca.bpn.gov in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. The following contract clauses apply to this acquisition and the attached clauses and provisions: FAR 52.212-4, Contract Terms and Conditions Commercial Items FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders The following subparagraphs of FAR 52.212-5 are applicable: All quoters shall submit the following: 1 copy of their quote to Donald Butler, Contracting Officer via email, Donald.Butler3@va.gov. This is an open-market combined synopsis/solicitation for services as defined herein.    The government intends to award a firm fixed price BPA, with a 1 year period of performance, as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoter shall list exception(s) and rationale for the exception(s). Submission shall be received not later than 11:00am est. on 9/04/2018 through email. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
