Page 2 of 3 Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-95, dated January 19, 2017. Solicitation 36C26218U0010 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued under as a Total Small Business Service Disabled Veteran Owned Small Business Set-Aside. NAICS Code is 339112 and the Small Business Administration s (SBA) size standard 1000 Employees. The Government is seeking a contract to acquire Verathon Glidesope Video Laryngoscopes and components. The equipment will be used in the hospital where a critical airway manipulation will be needed in an emergency situation. It will minimize the risk to veterans and to continue positive outcomes with how the laryngoscope blade is maneuvered in the posterior Pharynx and the Epiglottis is manipulated to allow successful visualization of the glottic opening during tracheal intubation. This solicitation contains a brand name justification for the following systems and components: SALIENT CHARACTERISTICS Must be designed for targeting difficult airway management. Must have active physical heating of the viewing lens Must have blade angle, for all patient sizes, Single Use and Reusable scopes must have the same profile Must have One-piece design BRAND NAME INFO: VERATHON 0270-0925 GS TI SPECTRUM, SU, MOBILE STAND 0570-0368 GLIDESCOPE GO MONITOR KIT Place of Performance: VA Long Beach Healthcare System (VALBHS), 5901 E 7th St Bldg 149 Long Beach, CA 90822. Period of Performance: Within 30 days of placing order or sooner if available. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process.
