- This is NOT a solicitation or request for quote (RFQ). This is NOT a request for proposal (RFP). This is NOT a job advertisement for hiring employees. The VA is seeking QUALIFIED VENDORS to provide a very brief response to this notice including only just what is asked for in this notice below. This is sources sought notice to industry and the only information is posted on FBO web page. Sources sought means seeking qualified vendors. The attachments included on the FBO page do not include any additional information. If your company is interested please read this notice entirely and submit the required information, as requested herein. VA is seeking current qualified sources ( vendors ) who have interest and capability of performing Lab Analysis of Samples from Dental Unit Waterlines to test for bacteria (colony count, e coli, coliform etc.) and provide reports. Quantities average 175 tests every 3 months. A qualified vendor must have the capability to include provision of Mail-order Testing Kits for use by VA Dental Assistants. VA Dental Assistants would test the Dental Unit Waterlines then ship the test results to the laboratory for analysis and reporting. A qualified vendor would have the capability to provide timely and immediate reports following receipt of test samples from the Dental Clinics. Results of reports must be issued in a timely manner to assist with informing the Dental Clinic, namely the VA Infections Control Officer, so they can take immediate action and corrective measures to ensure water safety and hygiene, and reducing risks to VA patients. Services are for Dental Clinics at Oklahoma City VA Medical Center. Additional qualifications include recent experience (within past 3 years) of performing services of a comparable size, scope and complexity, as well as current and valid registration in SAM.gov.
