Description

Statement of Need This is a Brand Name or Equal procurement. Title of Project: Respirator Fit Test Scope of Work: The vendor shall provide all resources necessary to accomplish the deliverables described in this Statement of Need (SON), except as may otherwise be specified and deliver to the desired location within the timeframe specified. Type of Contract: Firm Fixed Price Period of Performance: Delivery Date within 30 days of Date of Award unless otherwise coordinated with the Point of Contact (POC). General Requirement: The Wilmington VA Medical Center has a need for a Porta-Count Model 8048 along with MSPRO-4 Tablet, 8025-16 Fit Test Adapter and 8025-N95R Fit Test Probe Refill Kit. This is required to provide quantitative fit testing for medical staff wearing an N95 Respirator to work in direct patient care in areas where patients are impacted by airborne illness/disease. Delivery: 1. Contractor shall provide delivery of a PortaCount Model 8048 along with MSPRO-4 Tablet, 8025-16 Fit Test Adapter and 8025-N95R Fit Test Probe Refill Kit, to the Wilmington VA Medical Center. 2. Please contact the Point of Contact to coordinate time for delivery within (30) calendar days of the contract award unless otherwise directed by the Contracting Officer (CO) or Point of Contact (POC).
