THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI   53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Large business firms, to provide eight (8) AriaTele Telemetry Transmitter, brand name (Spacelabs 96281-C ) or equal, for the VA Illiana Health Care Center in Danville, Illinois. General Requirements: BRAND NAME (Spacelabs AriaTele Transmitter with display Item #96281-C) OR EQUAL: Salient Characteristics: Salient Characteristics: Telemetry Monitor Display System systems must consist of the following items: Telemetry monitor system must have 2 22 inch monitors with higher resolution touchscreen Telemetry monitor must be able to display ECG (Electro Cardio Gram) and SP02 (Saturated Oxygen) Telemetry monitor must be able to receive ECG and SP02 information from 16 patients at one time Telemetry monitor system must have 8 telemetry transmitters that will receive ECG/SP02 information from patients and record to CPRS (Centralized Patients Records System) Telemetry monitor system must have 8 receivers that must receive and record vital signs from patients to CPRS (Centralized Patients Records System) Telemetry monitor system must be able to display patients information on 2 each 22 inch monitor touch screen Telemetry monitor systems must have 1 each laser-jet printer with 110 voltage Telemetry monitor system must have 8 sets of 5 lead bundles that attach to patients All interested firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL THE FOLLOWING: 1. Company name. 2. Address. 3. Point of contact (name/title/telephone number/e-mail address). 4. FSS / SEWP Contract number, if requirement is on schedule. 5. DUNS number. 6. Tax ID number. 7. Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. 8. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above. 9. Documentation from manufacturer (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer. Submit responses to the Contracting Officer, Julie LeCourt, at julie.lecourt@va.gov no later than 4:30pm a.m. (Central Standard Time), 23 August 2018. 36C25218Q9802 Telemetry Transmitter for VAIHCS shall be referenced on all correspondence regarding this announcement. The NAICS Code 339112, All Other Plastics Product Manufacturing, is applicable to this acquisition; the size standard is $1million. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice; no other copies of this notice will be provided.
