Description
This is a Request for Information this is not a Request for Quote. The purpose of this notice is to conduct market research and obtain information from qualified vendors with special interest. No solicitation is being issued now. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to an acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The appropriate NAICS code 339112 has a small business size standard of 1000 (please annotate different NAICS if applicable) The U.S. Department of Veterans Affairs, Network Contracting Office 2, and the VA Medical Center in Syracuse, NY anticipate a purchase order for the following: Pressure Mapping System 0001 1 each SYS-01-00002 ForeSite SS Tablet Wireless +2LX100:36.36.02-C 0002 2-each ASY-01-00004 LX100:40.40.02 Cleanable Sensor Sensing Area: 50cm x 50cm (20" x 20") Resolution: 12.7mm (1/2") Pressure Range: 5-200mmHg 0003 1-each ASY-01-00011 PX100:26.64.01 Sensor Sensing Area: 77.5cm x 203cm (31 1/2" x 80") Resolution: 31.7mm (1 1/4") Pressure Range: 0.14-2.7N/cm2 (0.2-3.87psi) The equipment must be/have: The ability to see and record elevated pressures on the seated area and in the back with sufficient enough sensing points to know the exact location of elevated pressures. Must include the following features: A sensor pad with 1296 pressure sensing points on an 18 x 18 sensor to achieve the highest resolution possible. Ability to show not only where the pressure is higher, but exactly what the pressure rating is. A sensor pad that is thin and flexible to conform to the chair s seat and back to give an accurate view of pressure distribution. Pads that are cleanable to eliminate need for disposable sleeves. Compliant with IEC-60601-1 standards and certified as cleanable to the AAMI TIR-30:2011 standard Sensor Pad that never needs to be recalibrated and is ISO/IEC 17025 Accredited Sensor Pad that has +/- 5% full scale accuracy 2 sensor pads so that the back and seat may be analyzed simultaneously. A Touch screen Tablet with a wireless sensor pack with software that allows the clinician to see the various pressures applied to the pads and be able to visualize those with higher and lower values in real-time images and have: 9.7 inch high quality LED Panel 1024 x 768 resolution Capacitative mulit-touch Ability to withstand a drop from 4 onto concrete without sustaining damage Display images in 2D or 3D HIPAA compliant with data encryption Allows notes to be added and attached at each treatment session. Delivery need to be made to the Syracuse VAMC Include a standard 3-year warranty. If the offeror is quoting/offering or equal items, the offeror must clearly identify the exact make, model, manufacturer and item description (and picture) for each line item to be considered. Quotes/offerors that do not provide an adequate description of how the equal product meets the salient characteristics will not be considered. This procurement would be for new items only; no remanufactured or "gray market" items. FOB - destination Interested vendors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract. Documentation that confirms the company is registered and is the required socio-economic category in the System for Award Management (SAM). Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran-Owned Small Business (VOSB) offerors must be verified by the Department of Veterans Affairs Center for Verification and Evaluation (CVE) at the time of submission of proposal. Failure to be verified will result in the offeror s proposal to be deemed non-responsive. Responses should address how you will meet the Government's needs and provide a brief capability statement.
