- Attachment #2 J&A Attachment #1 The Department of the Army, ACC-APG, Natick Contracting Division, Natick, MA intends to purchase one (1) da Vinci® Xi TM Dual Console System (per attachment #1). This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Request For Quotation (RFQ) number is W911QY-18-Q-0214. This is a sole source requirement and award is intended to be made to Intuitive Surgical Inc. because they are the original equipment manufacturer (OEM) and there are no resellers or distributors. Seller must accommodate for a trade-in of one (1) da Vinci S System. The da Vinci® Xi TM Dual Console System is the standard robotic surgical system utilized in Department of Defense (DoD) Medical Treatment facilities and across the San Antonio Military Health System (SAMHS). The da Vinci® Xi TM Dual Console System is in support of training and certifying students, residents, and fellows in the 59th Medical Wing, Clinical Research Division's (59 CRD) Graduate Medical Education (GME) program to produce fully trained surgeons in the use of advanced surgical robotic systems currently in the DoD inventory. No substitutes are allowed. New items are required; refurbished/used items will not be accepted. All other timely offers will be considered. Offers for other products must contain complete product information, trade-in, and prices. This requirement is intended to be sole-sourced to Intuitive Surgical Inc., who is a large business. NAICS Code is 339112 and Size Standard is 1,000 employees. The Army's Office for Small Business Programs concurs with the sole-source determination. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. FAR 52.212-1 Instructions to Offerors--Commercial Items applies. The basis for award is Low Price Technically Acceptable (LPTA). Selection will be made based on the technically acceptable proposal with the lowest evaluated price. Past Performance will not be evaluated. Contractors must be registered in the System for Award Management (SAM) (https://www.sam.gov/portal/public/SAM/) to be eligible for award. Instructions for registration are available at the website. FAR 52.212-4, Contract Terms and Conditions--Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, and DFARS 52.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items apply. Any additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices shall apply. The following addenda or additional terms and conditions apply: In accordance with DFAR Clause 252.211-7003, Unique Item Identifier (UID) applies if the proposed unit price is $5,000 or greater. The UID information must be submitted in WAWF and a UID tag must be affixed to this item prior to shipping. Offers should include price and delivery terms and the following additional information: Cage Code. Offers may be e-mailed to Eileen Emond at eileen.emond.civ@mail.mil. Required delivery is No Later Than (NLT) 30 days after receipt of order (ARO). F.O.B. Destination 59th Medical Wing (AETC) Joint Base San Antonio- Lackland Texas.
