Description
Purchase Description Combined Synopsis/Solicitation This Modification/Amendment is to correct the Description of High Quality Planar Multi-Electrode Array for Retinal Recordings. It is now being advertised as COMBINED SYNOPSIS/SOLICITATION (COMPETITIVE) and the due date has been extended to August 23, 2018 at 12:00 PM Eastern. ALL OTHER PARTS OF THIS NOTICE, INCLUDING THE PURCHASE DESCRIPTION AND REQUIREMENTS, REMAIN UNCHANGED. COMBINED SYNOPSIS / SOLICITATION COMPETITIVE Title: High Quality Planar Multi-Electrode Array for Retinal Recordings (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is NIHDA-2018-00336 and the solicitation is issued as a Request for Quotation (RFQ). This acquisition is for a commercial item or service and is conducted under the authority of the Federal Acquisition Regulation (FAR) Part 13-Simplified Acquisition Procedures; FAR Subpart 13.5- Simplified Procedures for Certain Commercial Items; and FAR Part 12-Acquisition of Commercial Items, and is not expected to exceed the simplified acquisition threshold. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-99 dated June 15, 2018. (iv) The associated NAICS code 334516 and the small business size standard of 1,000 employees. This requirement is full and open with no set-aside restrictions. (v) Contractor shall provide the following equipment: 1. MaxOne System (MaxWell Biosystems Product No. MX1-SYS), including: a) Software MaxLab Live Basic, perpetual license, b) Support for 256 recording channels, and c) Support for 4 fixed electrode configurations. 2. MaxOne Chip Starter Package (MaxWell Biosystems Product No. MX1-CHP-START), including: a) Three (3) MaxOne Chips MX1-U-CHP, b) 1st Year MX1-SUB with 12 MX1-S-CHP, and c) Available only when purchased together with MX1-SYS. 3. MaxOne Recording Unit (MaxWell Biosystems Product No. MX1-BRD), including: a) Up to 4 units per system. 4. Recording PC (MaxWell Biosystems Product No. MX-PC), including: a) Recording computer with MaxLab Live pre-installed, b) Intel Core i7, 16GB DDR4, HDD 4TB 7200 RPM SATA, c) NVIDIA NVS 310 1GB 2xDP, 2nc GbE NIC, 24-inch monitor, and d) Warranty for MX-PC is two (2) years. 5. On-site training (MaxWell Biosystems Product No. TRN-ONSITE), including: a) System setup and installation, b) Training and assisted tutorials, and c) One day on-site (including travel and lodging expenses). 6. MaxLab Live Software Module for Routing (MaxWell Biosystems Product No. MX1-SW-ROUTE), including: a) Option to enable custom electrode to channel routing, and b) Allows generating custom electrode configurations. 7. MaxLab Live Software Module for 1,024 channels (MaxWell Biosystems Product No. MX1-SW-CHAN), including: a) Option to enable parallel streaming of 1,024 channels. 8. Tissue Holder for MaxOne (MaxWell Biosystems Product No. MX1-HLD), including: a) Micromanipulator to hold tissue preparations in place, b) Including bottom plate to mount on microscope stages, and c) Includes 2 replaceable inserts (MX1-HLD-INS). 9. Insert for Tissue Holder (MaxWell Biosystems Product No. MX1-HLD-INS), including: a) Inserts for using with MX1-HLD, b) Material PMMA, c) Includes holes for perfusion tubes, and d) Grid is not included and can be mounted with either a rubber band or glue. 10. Shipping Fee (MaxWell Biosystems Product No. SHP), including: a) Does not include customs clearance, duties and taxes, and b) Incoterms: DAP (vi) Brand Name or Equal Purchase Description: Product's Brand Name: MaxOne System Manufacturer's Name: MaxWell Biosystems AG (vii) Delivery is required within 90 days following contractor receipt of award. Equipment shall be delivered to the NIH Main Campus, 35 Convent Drive, Building 35A, Room 2E480, Bethesda, MD 20892. (viii) The provision at FAR clause 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. (ix) The provision at FAR clause 52.212-2, Evaluation - Commercial Items, applies to this acquisition. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Refer to attached Purchase Description for salient characteristics. (x) A completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications - Commercial Items (January 2017), is required with any offer submitted. This requirement may be met by completion of the provision in the System for Award Management. (xi) The FAR clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. (xii) FAR clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. (xiii) The following additional contract requirement(s) or terms and conditions as determined by the contracting officer are necessary for this acquisition and consistent with customary commercial practices. There are no additional contract requirement(s) or terms and conditions applicable to this acquisition. (xiv) The Defense Priorities and Allocations System (DPAS) are not applicable to this requirement. (xv) Responses to this solicitation must include sufficient information to establish the interested parties' bona-fide capabilities of providing the product or service. The price quote shall include: unit price, list price, shipping and handling costs, delivery days after contract award, delivery terms, prompt payment discount terms, F.O.B. Point (Destination or Origin), product or catalog number(s); product description; and any other information or factors that may be considered in the award decision. Such factors may include: past performance; special features required for effective program performance; trade-in considerations; probable life of the item selected as compared with that of a comparable item; warranty considerations; maintenance availability; and environmental and energy efficiency considerations. Respondents proposing on an equal product of the brand-name product specified in this announcement must provide, as part of their response: (a) product, catalog, model, and/or part number(s); (b) product description; (c) all relevant information and documentation that the item(s) offered meets the salient physical, functional, or performance characteristics as specified in the purchase description; quantity; estimated price or cost; shipping, handling, and/or installation charges; and delivery date after receipt of order. In addition the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size must be included in the response. All offerors must have an active registration in the System for Award Management (SAM) www.sam.gov." All responses must be received by August 23, 2018 at 12:00 PM Eastern Time and reference number NIHDA-2018-00336.
