Description
COMBINED SYNOPSIS SOLICITATION (i) This is a combined synopsis/solicitation for Welch Allyn Connex Spot Monitors - BRAND NAME, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24418Q9585 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS 339112 code has a small business size standard of 500. Contract Title. Purchase, installation and set up of Welch Allyn Connex Vital Signs Machines 2. Background. The Coatesville VA Medical Center currently has 120 Welch Allyn Connex Spot Monitors that wirelessly talk with the Connex Server to send vital signs to CPRS. Five of these devices are currently at our community based outpatient clinic (CBOC) located in Springfield, PA. This CBOC is moving to a new location and therefore, more vital sign machines are needed to accommodate for the increase in exam rooms. 3. Scope. Contractor is to provide, install and set up 9 Welch Allyn Connex Spot Monitor with SureBP Non-Invasive Blood Pressure, Covidien SpO2, Sure Temp Plus Thermometer at the VA Medical Center, Coatesville, PA. 4. Specific Tasks. The contractor shall provide, install, and set up the following vital signs machines: 9 Welch Allyn Connex Spot Monitor with SureBP Non-Invasive Blood Pressure, Covidien SpO2, SureTemp Plus Thermometer 9 Welch Allyn Connex Spot Monitor Accessory Power Management Stand 9 Barcode Scanner with Holder 9 Welch Allyn Device Connectivity License 9 5-year Connex Spot Monitor, Partners in Care Comprehensive Partnership Program with Annual Calibration Service The contractor shall provide onsite installation to include: de-palletizing shipment, unboxing and staging product, unboxing and assembling roll stands, attaching device to stand, attaching accessories to device (SpO2 cable, NIBP hose and cuff, temperature sensor, power supply, barcode scanner, quick reference card), performing device in-service, disposing of waste packaging, configuring devices to the latest configuration file, and providing summary report to customer. All units must communicate with the Connex Integration Software on the Welch Allyn Connex Server. Contractor will set up and install each vital signs machine including wireless transmission as needed. All units must send the patient vital signs to the patient record (CPRS) through the Welch Allyn Connex Server. 5. Performance Monitoring All units purchased will be subject to an incoming inspection by Biomedical Engineering to ensure the units are functional and meet electrical safety standards as defined by the current edition of NFPA 99. 6. Security Requirements. The Contractor will be installing and setting up medical devices which can contain protected health information. The Contractor will not routinely access this information but may have to do so in the course of installing the medical device. The Contractor will not disclose this protected health information. The C&A requirements do not apply and a Security Accreditation Package is not required. In accordance with the Assistant Secretary for Information and Technology Memorandum dated Mar 24,2011, the following requirements must be met: - A prohibition on unauthorized disclosure: See VA Handbook 6500.6, Appendix C, paragraph 3.a. - A requirement for data breach notification: See VA Handbook 6500.6, Appendix C, paragraph 6.a. - A requirement to pay liquidated damages in the event of a data breach: See VA Handbook 6500.6, Appendix C, paragraph 7.a, 7.d - A requirement for annual security/privacy awareness training: See VA Handbook 6500.6, Appendix C, paragraph 9 [all] - A requirement to sign VA's Rules of Behavior: See VA Handbook 6500.6, Appendix C, paragraph 9, Appendix D - Contract personnel for Maintenance/Installation Contracts are exempted from paragraph 2(B), if a BUSINESS ASSOCIATE AGREEMENT (BAA) is in place. If a BAA is not in place, paragraph 2B applies. - Maintenance/Installation contracts for Bio-Medical Devices should include wording from VA Handbook 6500.6, Appendix C, Paragraph 5.h (all), including all sub-paragraphs down through 5.h.(4)(c) If the procured system requires software to be installed on a VA computer (client) which is not a medical device, the software will need to be approved on the One-VA Technical Reference Model (TRM) and conform to any applicable constraints. PO Comments: A BAA is required as contractor will/may have access to VA sensitive information. A national BAA is already in place with Welch Allyn, Inc.. The contractor is to review the BAA along with the contract. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There is no GFE or GFI anticipated with this contract. 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. There is no follow-on work anticipated. b. Identification of Potential Conflicts of Interest (COI). None identified. c. Identification of Non-Disclosure Requirements Contractor will not disclose any ePHI that may be viewed incidental to installing the purchased equipment. d. Packaging, Packing and Shipping Instructions. There are no special requirements. e. Inspection and Acceptance Criteria. As previously noted, all units purchased will be subject to an incoming inspection by Biomedical Engineering to ensure the units are functional and meet electrical safety standards as defined by the current edition of NFPA 99. 9. Risk Control. Contractor is responsible for providing all applicable training and protective equipment to protect Contractor s employees from any hazards, including blood borne pathogens, which may be encountered in the performance of this contract. 10. Place of Performance. All work will be performed at the VA Medical Center, 1400 Black Horse Hill Road, Coatesville, PA, 19320. 11. Period of Performance. Contractor will have two (2) months from contract award to complete the installation. 12. Delivery Schedule. Contractor will set delivery schedule to ensure that installation can be completed within two (2) months from contract award. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and the following clauses AND instructions are added as addenda: CLAUSES: 52.214-21 Descriptive Literature (APR 2002) 852-219-10...
