- FA480318R0018A001 Amendment Document (SIGN THIS DOCUMENT) Questions and Answers Updated PWS correcting errors and missing data identified in Questions and Answers. FA480318R0018A001 Conform Document (USE THIS FOR YOUR PROPOSAL SUBMISSION) Amendment 0002: ***Important! Please read this Amendment in full Improper completion can result in a proposal being found non-responsive!*** Note to offerors: The Amendment Document reads Amendment 0001 as assigned by our contract writing system and cannot be modified. This is due to Amendment 0001 of the FBO Solicitation still containing the original Solicitation Document due to the posting of the incorrect Solicitation Draft Document by Contracting in the original FBO post. Therefore only one Amendment has been created in the contracting writing system, but this FBO Solicitation has two Amendments. For simplicity and to avoid confusion, from here on out, The Original FBO Posting and Amendment 0001 on FBO posting the correct Solicitation Document shall be referred to as the "Original Post". The Amendment being posted TODAY, 15 Aug 2018, shall be referred to as "Amendment 0001" as stated in Block 2 of the Form SF30 Amendment Document. The Purpose of this Amendment is to: 1.The Questions and Answers has been incorporated as Attachment 6 of the RFP 2. Change the Due Date for Proposals from 16 August 2018 to 21 August 2018 @ 2:00p.m. EDT to afford prospective offerors the time to review the Q&A and the changes to the CLINs as stipulated in the Amendment Document and Q&A. 3. Add a fixed price/rate based on publicly available rates to the compacted trash (CLIN X002/$42.25), Bulk Trash (CLIN X005/$33.00), and Organic Waste (CLIN X012/$25.00) tipping fees. Note: Offerors will not attach a price to these CLINs. 4. Incorporate Performance Work Statement dated 14 Aug 2018. Changes to the PWS include correcting an error in Paragraph 1, adding Scrap Wood and Tires to Table B1, adding Organic Waste Carts to Appendix A, and changes in wording in paragraph 1.6.1 and 1.6.3. Attachments 2-5 of the "Original Post" remain unchanged other than the PPQs are now not due until 21 August 2018 at 2:00pm EDT. PPQs do not need to be re-requested from references, and any PPQs received remain valid. Offerors may inform parties they know have not submitted they have until 21 August 2018 at 2:00pm EDT to submit the PPQ to Contracting. ***Signed Acknowledgment of "Amendment 0001" that was posted on 15 Aug 2018 in block 15 A,B,and C is required to be submitted with your proposal. Failure to do so will result in the proposal being found non-responsive.*** ***Ultilize the conform copy of the Solicitation uploaded in "Amendment 0001" to complete your proposal for your submission.*** All Other Terms and Conditions Remain Unchanged.
