- THIS IS A SOURCES SOUGHT NOTICE. This is not a request for quotes. A solicitation will be issued at a later date. The Department of Veterans Affairs, South Central VA Health Care System (VISN 16) has a requirement for an authorized distributor of Bio-Rad ® products to provide Bio-Rad ® Quality Control (QC) material to include lot number standardization and lot sequester for VISN 16 Medical Centers and Outpatient Clinic Laboratories. The intent of the projected RFQ is for the award of a firm-fixed price BPA (Blanket Purchase Agreement) for a base year (October 1, 2018 through September 30, 2019) plus 4 one-year options. VISN 16 locations are identified in Attachment A. SCOPE: The Contractor shall be responsible for assuring that an adequate supply of BPA products are available to meet the requirements. With few exceptions, deliveries of BPA items shall be made the next business day (Tu-Fri) when orders are placed M-Th by the specified daily cut-off hour. Deliveries shall be within regular work hours, which are usually 8:00 AM to 4:30 PM, Monday through Friday, for Medical Center Laboratories. Deliveries to Outpatient Clinic Laboratories shall be made during their hours of operation but no later than 30 minutes prior to daily close of business. Note: Hours may vary at different facilities. Shipping/handling charges, with the exception of those for hazardous materials (Haz-Mat) shall not be applied to any single order totaling $250.00 or more. These items will be of the highest quality, sensitive, specific and tested to assure precision and accuracy. Following BPA award, unexpected changes in methodology/technology will be at the expense of the contractor. Alert/Notification of any delays in shipment as well as any of all technical advisory/recalls/alerts, prior to or simultaneously with field alerts should be forwarded to the laboratory points of contact and the VISN 16 Contracting Officer s Representative (COR). TECHNICAL REQUIREMENTS Contractor shall have a valid, signed Vendor authorization letter for distribution of the required Bio-Rad Quality Control products from the manufacturer. Dedicated/designated staff shall be assigned to: manage the lot sequester so each VISN 16 facility is on the same lot at the same time, ensure VISN 16 receives courtesy shipments of new lots and adequate inventories are established based on usage and individual lot expiration date for each different control type. Only the brand and item numbers of Bio-Rad Quality Control Products specified is acceptable. Quotations for other brands or manufacturers of quality control products will be found unacceptable. On an as-needed basis, participating facilities will order control product. Pertinent information, including quantities required, delivery schedule and billing instructions will be provided. These items shall be of the highest quality, sensitivity, specificity and tested to assure precision and accuracy. Expiration date must be clearly marked on reagent, standards and control containers. In the event that material defect is suspected and/or detected by the Contractor will be requested to provide any consultation necessary to alleviate any said suspicion and/or defect by the following: a. Provide technical expertise which may include professional examination of product; results and recommendations shall be sent to the Contracting Officer. b. Make expedient replacement of any product suspected of being deficient. Contractor shall sequester specific lots of controls to ensure all ordering locations are using the same lot of controls. Contractor shall appropriately and effectively manage inventories to prevent product waste. Contractor may relocate product quantities within the network laboratories to enhance standardization of lot numbers at a given time. Impacted laboratories will be informed timely of any adjustments. Contractor shall furnish monthly updates by facility on inventory availability of sequestered lots. Contractor shall notify respective laboratories of removals/reductions of lot sequesters prior to monthly updates when significant changes occur and when quantities are reduced to zero. Contractor shall offer maximized expiration dates for control lots to reduce frequency of new lot validations and/or parallel cross over studies. Minimum lot dating will be provided. Contractor shall supply courtesy samples of upcoming lot of each control to sites prior to expiration or depletion of sequester inventory for performance of lot validations and/or parallel cross over studies. Shipment timing shall allow for completion of appropriate studies to meet accreditation requirements in adherence to individual lab policy/procedure. Contractor shall provide recall notification, regardless of level, to each facility, and to the VISN 16 COR. Contractor shall include a listing of all Bio-Rad control products listed in its FSS schedule; FSC 65 VII; SIN 555-2. Medical Centers shall be notified immediately of any backorder item(s). Contractor s failure to provide any item, contained herein within the specified delivery time shall allow a Medical Center to obtain the product from another source. No substitutions will be permitted without approval from the Contracting Officer or his/her designated representative. The applicable NAICS code is 325413 In-Vitro Diagnostic Substance Manufacturing and the size standard is 1,250 employees. The resulting BPA will have fixed unit costs with estimated annual quantities for the period of Oct 1, 2018 through Sept 30, 2019 plus 4 one-year option periods (10/1/2018 thru 9/30/2023). It is anticipated that a Request for Quote will be issued in September 2018. Award of a firm-fixed price BPA is contemplated by October 1, 2018. This notice is published to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Businesses (LB) capable of performing the requirements to warrant full and open competition. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance of reagent orders; (3) FSS/GSA Contract information; (4) Authorized distribution/dealer letter from Bio-Rad ® and (5) any other pertinent company documentation. The response date to this notice is August 16, 2018 at 3:00 (central time). This market research is for informational and planning purposes only.
