- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: quotes are being requested and a written solicitation will not be issued. Solicitation number W91YTZ-18-Q-0189 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-97 and Defense Federal Acquisition Supplement Publication (DPN) 20180530. This acquisition is issued on an UNRESTRICTED FULL AND OPEN basis under NAICS code 333318 and size standard is 1000. The Government anticipates awarding a single award issued as Firm Fixed Price. The requirement is to provide a base plus 4 year option periods for Nephrology Bacteria Testing and LAL testing on samples provided from reverse Osmosis (RO) machines and Hemodialysis machines and perform AAMI analysis on samples provided from RO machines as well as city tap water for Dwight D. Eisenhower Army Medical Center, Fort Gordon, Georgia. All responsible sources may submit a quotation which shall be considered by the agency. Instructions for the Preparation of Proposals The technical information required for evaluation purposes shall not exceed 10 pages. Each page shall be numbered. The proposal shall be prepared in two (2) separate volumes as indicated below. ~ Volume I-Technical ~ Volume II-Pricing VOLUME I - TECHNICAL The technical proposal, Volume I, shall be addressed in sufficient written detail for the Government to determine if the offeror understands this aspect of the Government's requirement. The offeror shall address each factor separately, and shall provide sufficient narrative and supporting data for each factor. Lack of any of these significant capabilities shall be considered sufficient cause for the proposal to be determined unacceptable. The Technical Proposal shall provide sufficient detail for the Government to determine whether the proposal acceptably meets the requirements of the solicitation. Statements that the offeror understands, can or will comply with all specifications, or statements paraphrasing the specifications of parts thereof, or phrases such as "standard procedures will be used" or "well-known techniques will be used" will be considered unacceptable. The technical proposal shall provide a narrative and supporting data with the following sections organized separately so that the evaluation factors listed below are included. VOLUME II - PRICING Assessment of price reasonableness based on comparison to market prices. a. The offeror shall submit proposed pricing for all subcontract Line Item Numbers (subCLINs) identified in the Request for Proposal, unless otherwise noted. The offeror shall limit their unit and extended prices to two (2) decimal places (example: $158.03). b. The offeror shall submit pricing totals for the base period and option years individually, as well as a grand total. Addendum 52.212-2 (LPTA): Paragraph (a) replaced with the following: Award will be made using the lowest price technically acceptable (LPTA) source selection process. Award will be made to the responsible offeror on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. Therefore, each initial offer should contain the offeror's best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if it is later determined by the contracting officer to be necessary. In order to be considered for award, an Offeror must receive an Acceptable rating in all factors. The following factors shall be used to evaluate: Technical and Price Award will be made based on lowest price, technically acceptable proposal. (a) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contractor without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. FACTOR: 1 TECHNICAL PROPOSAL Certification - Shall be provided in writing, or documentation of your company maintaining compliance with TJC Infection Control Requirements. Shall be able to perform testing weekly for first 4 weeks and at a minimum monthly thereafter. Shall provide 24/7 access to the customer results. The Colony Count testing will be reported on Sundays, Holidays, or any day that testing is completed. Technical will be evaluated as either Acceptable or Unacceptable. Best value is expected to result from selection of the technically acceptable proposal with the lowest evaluated price. Definitions are as follows: ACCEPTABLE: To be rated acceptable, the offeror's technical proposal must demonstrate it can meet all specifications and testing requirements. UNACCEPTABLE: A technical proposal that fails to demonstrate it can meet one or more requirements will be determined unacceptable. FACTOR II: PRICE Price will be evaluated separately from Technical and will be evaluated for fairness and reasonability. The offeror shall complete the CLIN structure with a total cost covering a base period and three option years. The contractor shall also perform AAMI analysis on samples provided from RO machines as well as city tap water. 1.2 PERFORMANCE REQUIREMENTS: Contractor shall be able to perform testing weekly for first 4 weeks and at a minimum monthly thereafter. 1.2.2 PERIOD OF PERFORMANCE: Base year plus four (4) option years 1.3 OWNERSHIP OF EQUIPMENT 1.3.1 The 4 WRO H 300 dialysis reverse osmosis systems and the 4 Fresenius 2008T Hemodialysis machines (HD) are the property of the United States Army. 1.4 BACKGROUND INFORMATION 1.4.1 The Contractor shall comply with Federal, State and local laws and regulations applicable to the performance of this contract. 1.4.2 The Contractor shall not accept any instructions issued by any person other than the Contracting Officer, or the Contracting Officer's Representative (COR) acting within the limits of their authority. 1.5 PERSONNEL 1.5.1 Program Manager: The Contractor shall provide, in writing, the name and phone number of a Program Manager within (10) ten calendar days of the award of the contract. The contractor's Program Manager shall correspond with the Contracting Officer's Representative (COR) on a regular basis to discuss any problems that the contractor or contractor's personnel may be experiencing during the performance of this contract. Unresolved problems shall be referred to the contracting officer for resolution. 1.5.2 Government Point of Contact (POC). The COR will be the Government's POC. The COR will be designated in writing to the Contractor and the scope of authority will be set forth therein. Contractor shall respond only to calls from COR or a designated representative from the Ophthalmology Service. 1.6 PHYSICAL SECURITY 1.6.1 If necessary, contractor personnel or any representative of the contractor entering Ft. Gordon shall abide by all security regulations and shall be subject to security checks. When entering post, contractor personnel must have vehicle registration, driver's license and proof of insurance. 1.7 QUALITY CONTROU QUALITY ASSURANCE 1.7.1 The contractor shall be responsible for ensuring that the quality of service and materials provided under this contract meet or exceed the statement of work. 1. 7.2 Upon completion of work, an acceptance inspection will be conducted by the COR and the equipment user to ensure all services are satisfactory and completed. 2.0 ACRONYMS AND DEFINITIONS 2.1 Contracting Officer (CO). A person duly appointed with the authority to enter into and administer contracts on behalf of the U.S. Government. 2.2 Contracting Officer's Representative (COR). A Government employee selected and designated in writing by the CO to act as their authorized representative in administering a contract. The Government POC or their designated representative at the equipment work site has the authority to request services and performance of the contractor against his contract. 3.0 SPECIFIC TASKS 3.1 Quality assured testing services on samples provided by DDEAMC from 4 reverse osmosis machines, 4 hemodialysis machines and city tap water. 3.1.1 The Contractor shall provide quality-assured Limulus Amoebocyte Lysate (LAL) testing and bacteria cultures on samples provided by DDEAMC from 4 reverse osmosis and 4 hemodialysis machines weekly for the first the first four weeks. 3.1.2 The Contractor shall provide quality-assured LAL testing and bacteria cultures on samples provided by DDEAMC from 4 reverse osmosis and 4 hemodialysis machines monthly thereafter. 3.1.3 The Contractor shall provide Association for the Advancement of Medical Instrumentation (AAMI) analysis on samples provided by DDEAMC from 4 reverse osmosis and 4 hemodialysis machines annually. 3.1.4 The Contractor shall provide results of testing to appropriate staff at DDEAMC monthly. 4.0 PERFORMANCE REQUIREMENTS PERFORMANCE REQUIREMENTS MATRIX Paragraph Requirement(s) AQL 1.4.1 Compliance with Federal, State and local laws and requlations.
