Description
This is a combined synopsis/solicitation for a commercial item, for a Full Field Laser Perfusion Imager (Laser Speckle), prepared in accordance with the format in FAR Subpart 12.6, as a Request for Quote and supplemented with additional information included in this notice. The solicitation number is 36C24E18Q9518. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-99. The associated North American Industrial Classification System (NAICS) code for this procurement is 333314. The attached solicitation document contains all line item numbers, quantities, units of measure, descriptions of requirements for the items to be acquired, dates and places of delivery, along with other applicable specifications. This is a Brand Name or Equal requirement (see SOW for details, Item # MOORFLPI2, Full Field Laser Perfusion Imager (Laser speckle)). This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. The selected offeror must comply with commercial item terms, conditions and provisions, which are incorporated in the attached solicitation document. The full text of the referenced FAR clauses may be accessed electronically at https://www.acquisition.gov/far/. In addition, the offeror must review and acknowledge the attached solicitation document, item details, delivery schedule, and all other requirements, and agree to all terms and conditions set forth in the solicitation document. Quotes will be evaluated using Best Value procedures. This is a brand name or equal requirement per VAAR 852.211-73 Any questions pertaining to this solicitation must be submitted, in writing to the Contract Specialist, linda.buccigrossi@va.gov by 1:00 PM ET on Friday, August 10, 2018 and will be addressed via an amendment to the solicitation. All quotes shall be submitted to the Contract Specialist by email at linda.buccigrossi@va.gov by 12:00 PM (noon) ET on Wednesday, 8/15/2018. The email subject line should read: Solictation Speckle Laser No hardcopy proposals will be accepted, and late offers will not be accepted. This acquisition will utilize best value procedures. Comparative Evaluations will be conducted in accordance with FAR 13.106-2(b)(3). The Government intends to evaluate quotes IAW FAR Part 13.106-2(b)(3) using Comparative Evaluation procedures. The Government will select the quote deemed the best value when compared to all other quotes. The Government will use the value indicators listed below to determine the best value. Lowest price may or may not be deemed the best value. The Government will directly compare quotes to each other, considering the value indicators listed below. Direct Comparative Evaluations will be based on the following: Factor 1. Cost/Price Factor 2. A narrative addressing each item included in the solicitation, presenting clear evidence of ability of vendor s product to meet the salient brand name or equal characteristics contained in this solicitation. Factor 3. Past Performance Information Offeror must submit: Offeror must submit a cost proposal as a separate document (see section B.5, page 7 of this solicitation with/quote). Offeror shall provide a narrative to address each factor, presenting clear evidence of their ability to meet or exceed all requirements of this acquisition.
