Performance Work Statement Siemens Medical Equipment Maintenance Services 27 April 2018 New Mexico Veterans Affairs Health Care System (NMVAHCS) 501-19-1-055-0002 INTRODUCTION: This requirement is for Preventative Maintenance on the Siemens Medical Systems and Equipment currently in use throughout the New Mexico Veterans Affairs Health Care System in Albuquerque, New Mexico. BACKGROUND: Siemens Healthcare is the original equipment manufacturer (OEM), and sole vendor which is capable of servicing, repairing, and supplying replacement parts for the OEM equipment utilized by VISN 18 Medical facilities. Siemens Healthcare has also provided a written statement confirming that they are the only source certified to perform any maintenance or repair on their equipment, to include necessary proprietary software upgrades. CONFORMANCE STANDARDS The contractor will perform preventive maintenance (PM) inspection on equipment/systems as provided for in the contract agreement. PMs will be accomplished in accordance with the Original Equipment Manufacturer (OEM) recommendations. The contractor shall use OEM certified parts and service engineer/technicians must be OEM trained on all equipment included in agreement. Contract service shall ensure that the equipment functions in conformance with the specifications used when the equipment was procured by the VAMC, and any upgrades/updates, as well as following most current published standards/ specifications/regulations: Manufacturer's specifications, Association for the Advancement of Medical Instrumentation (AAMI), The Joint Commission (TJC), National Fire Protection Agency 99 (NFPA-99), Center for Device and Radiological Health (CDRH), Original Equipment Manufacturer (OEM), American Hospital Association (AHA), Institute of Electrical And Electronic Engineers (IEEE), Occupational Safety and Health Administration (OSHA), College of American Pathologists (CAP), Memphis VAMC standard operating procedures, and any other Federal, State, and Local regulations pertaining to the equipment listed for this contract. HOURS OF COVERAGE: Normal hours of coverage are Monday through Friday from 7:30 am to 4:00 pm Arizona time (no daylight savings) and 8:00 am to 4:00 pm during WHEN hours. All service/repairs will be performed during normal hours of coverage unless requested or approved by the COTR or his/her designee. Preventive maintenance will be performed as scheduled by the COTR or their designee. Preventive maintenance will be performed during normal hours of coverage. Work performed outside the normal hours of coverage must be approved by the COTR or his/her designee. Funding for services that are outside the normal coverage hours or on holidays must have approved funding. Federal Holidays observed by NMVAHCS are: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day SCHEDULED AND UNSCHEDULED 1"1AlNTENANCE (Emergency Repair Service) SCHEDULED MAINTENANCE 1. NMVAHCS Biomedical Engineering technician(s) shall perform scheduled PM's. Contractor shall provide to Biomedical Engineering Radiology Section recommended P.M. procedures and checklists with worksheet originals indicating work to be performed and actual values obtained, if applicable. Scheduled maintenance shall include an itemized list of the procedures performed, including electrical safety, and shall include, but need not be limited to, the following: Cleaning of equipment Reviewing operating system software diagnostics to ensure that the system is operating in accordance with Equipment Performance Calibrating and lubricating the equipment Performing remedial maintenance of a non-emergent nature. Testing and replacing faulty and worn parts and/or parts likely to become faulty, worn, or failed. Inspecting and replacing, where indicated, electrical wiring and cables for wear and fraying. Inspecting and replacing, where indicated, all mechanical components which may include, but is not limited to, X-ray tube mounting hardware; patient restraints and support devices; cables and mounting hardware, chains, belts, bearings and tracks, interlocks, clutches, motors, keyboards, and patient couches for mechanical integrity, safety, and performance. Returning the equipment to the operating condition as stated in Equipment Performance Providing documentation of services performed 11. Contractor shall perform one (1) Quality Assurance Review per year to ensure that equipment performs in accordance with Equipment Performance Section 2. The reviews shall be performed jointly by contractor and NMVAHCS Biomedical engineering technician(s) during the hours defined in the preventive maintenance schedule 111. Hardware/firmware/software updates/upgrades shall be scheduled and performed during normal hours of coverage. Manufacturer ­ approved virus protection with all required virus definition file updates shall be included for all systems listed in Section 4 1v. Contractor shall provide an outline of the Quality Assurance Review and scheduled maintenance to the COTR within 15 days after the contract is awarded. Any changes to the schedule shall be coordinated and approved by the COTR. UNSCHEDULED MAINTENANCE (Emergency Repair Service) Contractor and Biomedical Engineering shall maintain the equipment in accordance with Section 2 Equipment Performance. Contractor will provide, in addition to the Quality Assurance Review, unlimited telephone technical support and a labor bank of 120 hours for onsite service calls. These service calls may be used for any maintenance or repair service as determined by Biomedical Engineering Radiology Section. 11. Response Time: Contractor's FSE or Technical Support must respond by phone to Biomedical Engineering Radiology Section within one (1) hour after receipt of telephoned notification during normal duty 8am to 5pm Monday through Friday. If the problem cannot be corrected by Biomedical Engineering, the FSE will be on site within two (2) hours of notification. PARTS - ITEMS 1. The contractor shall furnish parts. The contractor shall maintain quantity of parts including unique and/or high mortality replacement parts in such a manner as to be available for shipment within 24 consecutive hours of request. All parts supplied shall be compatible with existing equipment. The contract shall include all parts with the following exceptions: replacement x-ray tube and flat detector(s). Contract parts coverage is to include all system options, third party pass thru devices, such as, compact disk devices, remote workstations, etc. and will include all Siemens supplied software including that used by third party devices. The contractor shall use new or rebuilt parts. All parts shall perform identically to the original equipment specifications. SERVICE MANUALS/TOOLS/EQUIPMENT 1. The NMVAHCS shall not provide tools, test equipment, service manuals, or service diagnostic software to the contractor. Contractor shall provide all tools, test equipment, service manuals, and/or service diagnostic software. Contractor shall obtain, have on file, and make available to its FSE all operational and technical documentation (such as operational and service manuals, schematics, diagnostic software, and parts lists) which are necessary to meet the performance requirements of this contract. The location and listing of the service manuals, by name and/or the manuals themselves shall be provided to the CO upon request. 11. Contractor shall provide the NMVAHCS Biomedical Engineering Radiology Section copies of all documentation that pertains to any hardware, software, or firmware upgrade or repair. Contractor shall provide to the NMVAHCS Biomedical Engineering Radiology Section copies of the latest version(s) of all technical documentation and licensing agreements, which shall include operational and service manuals, schematics, diagnostic software, and parts lists. NMVAHCS Biomedical Engineering Radiology Section shall have access to the same operation, service, and documentation as the OEM's FSE. Contractor shall provide to the NMVAHCS Biomedical Engineering Radiology Section OEM Service Bulletins for the equipment listed herein. DOCUMENTATION/REPORTS: At the completion of each service call (scheduled or unscheduled), Contractor/FSE shall provide a detailed ESR. The ESR shall, at a minimum, document the following information legibly and in complete detail: Name of contractor and contract number Name of the FSE performing services. Contractor Service ESR number/log number. Date, time (starting and ending), equipment downtime and hours on-site for service call. Identification of equipment to be serviced: I.D. number, Manufacturer's name, device name, model number, serial number, and any other Manufacturer's identification numbers. Description of problem reported by Biomedical Engineering Radiology Section. 1v. Signatures: FSE performing services described. Authorized VA Employee who witnessed service described. v. Any additional charges claimed must be approved by Biomedical Engineering Radiology Section before service has commenced. REPORTING REQUIREMENTS 1. Contractor shall be required to report to Biomedical Engineering Radiology Section to check in. This check-in is mandatory. The FSE shall check out with Biomedical Engineering Radiology Section and submit the ESR(s) to Biomedical Engineering Radiology Section. All ESR shall be submitted to Biomedical Engineering Radiology Section for an "acceptance signature". If Biomedical Engineering Radiology Section is unavailable, a signed, authorized copy of the ESR will be sent to Biomedical Engineering Radiology Section. VA Biomedical Engineering Radiology Section Contact Person(s): J. R. Duncan, Hugh Lowrie or Roger Phillips by phone at (505) 256-2726 or in Location (Room Number/Building): BCI00-41. The contractor shall provide COTR telephone number(s) to contact the Service Department or FSE PAYMENTS AND ADDITIONAL CHARGES 1. PAYMENTS: Invoices will be paid IN ARREARS monthly. (If the Contractor prefers quarterly, or semiannually, permission can be granted for other than by a monthly basis by the CO). The Contractor shall mail the invoices to the address provide on the Continuation Sheet of the SF 1449. Invoices, MUST INCLUDE, at a minimum, the following information: Contractor's name, address, phone number, contract or purchase order number, period of service the billing covers, list of equipment items covered during the stated period of service and invoice amount. Invoice shall be submitted for payment to: New Mexico VA Health Care System Engineering Service (138) 1501 San Pedro SE Albuquerque, NM 87108 11. REPORTING REQUIRED FOR SERVICES BEYOND THE CONTRACT SCOPE: The Contractor shall immediately, but not later than twenty-four (24) hours after discovery, notify the CO and the COTR, IN WRITING of the existence or the development of any defects in, or repairs required to the schedule of equipment which the Contractor considers not to be responsible for under the terms of the contract. The Contractor shall furnish the CO and COTR with a written estimate of the cost to make necessary repairs. 111. ADDITIONAL CHARGES: There shall be no additional charge for time spent at the site (during, or after the normal hours of coverage) awaiting the arrival of additional FSE and/or delivery of parts CONDITION OF EQUIPMENT: 1. Contractor shall accept responsibility for the equipment described in Part I in "as is" condition. Failure to inspect the equipment prior to contract award will not relieve Contractor from performance of the requirements of this contract. COMPETENCY OF PERSONNEL SERVICING EQUIPMENT: 1. "Fully Qualified" is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized training program, for the equipment identified in the Part I. For field experience, the FSE(s) has a minimum of two years of experience (except for equipment newly on the market) with respect to scheduled and unscheduled preventive and remedial maintenance, on equipment listed herein. 11. The FSE shall be authorized by Contractor to perform the maintenance services. All work shall be performed by "Fully Qualified" competent FSE. Contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSE for each make and model Contractor services at the NMVAHCS. The CO may authenticate the training requirements, request training certificates or credentials from Contractor at any time for any personnel who are servicing or installing any NMVAHCS equipment. The CO and/or COTR specifically reserve the right to reject any of the Contractor's personnel and refuse them permission to work on the NMVAHCS equipment. m. If subcontractor(s) are used, they must be approved by the CO; Contractor shall submit any proposed change in subcontractor(s) to the CO for approval/disapproval. TEST EQUIPMENT: 1. Prior to commencement of work on this contract, Contractor shall provide the NMVAHCS with a copy of the current calibration certification of all test equipment to be used by the Contractor on NMVAHCS equipment. This certification shall also be provided on a periodic basis when requested by the NMVAHCS. Test equipment calibration shall be traceable to a national standard. IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: 1. Contractor FSE shall wear visible identification (at all times) while on the premises of the NMVAHCS. It is the responsibility of Contractor FSE to park in the appropriate designated parking areas. Information on parking is available from the VA Police Service. The NMVAHCS will not invalidate or make reimbursement for parking violations of Contractor's FSE under any conditions. Smoking is prohibited inside any buildings at the NMVAHCS. Possession of weapons is prohibited. 11. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. INSURANCE: 1. Contractors are required to comply with applicable Federal and State Worker Compensation and occupational disease statutes. 11. Contractors are required to have Bodily Injury Liability Insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence. m. Contractors are required to have Property Damage Liability insurance coverage of at least $500,000. EQUIPMENT MODIFICATIONS/UPGRADES/UPDATES: 1. Equipment modifications are divided into two kinds; 1) Normal functional improvements that assure that the equipment is operating at the peak of performance and 2) Enhancing improvements are optional improvements that change the equipment's original design to allow for supplementary or additional functions. 11. Normal functional improvements shall be provided at no cost to VA within six (6) months after each version is released. Installation shall be done at no additional cost to the government during normal working hours, unless requested by the COTR m. Enhancing improvements are optional to VA. If the COTR has determined that VA requires such enhancing improvements, it shall be considered outside of the scope of this contract and processed as a separate procurement. 1v. All modifications, upgrades, updates, enhancements, etc., must be scheduled in advance with the COTR. CO and COTR must receive notice at least five (5) full business days in advance before any equipment updates, upgrades, enhancements, and modifications are begun along with justification (if Vendor-suggested modification), estimated costs (if applicable), probable effect on equipment operation, and needs for new or additional in-service training. Documentation must be provided to Biomedical Engineering Radiology Section when software upgrades/updates are installed. Documentation will include what was installed, how it was installed, and where it was installed on the system. Documentation will include parts list, calibration procedures, and test procedures, if applicable. SPECIAL CONDITIONS The Contractor shall support all system and third-party software of the equipment, providing onsite support and repairs during normal hours of coverage. The Contractor will provide telephone support during normal hours of coverage. The Contractor will provide provision revisions and updates of all software that corrects functional flaws. 11. Contract labor coverage to include all system options, third party pass thru devices, such as, compact disk devices, remote workstations, etc and will include all Siemens Medical Solutions USA, Inc. supplied software including that used by third party devices. DOCUMENTATION 1. Service reports will be provided for all service activities including preventive maintenance, emergency repair, and safety inspections, and must contain the following information: 11. Date(s) and time-period of service. 111. Complete description of equipment serviced including model number and serial number. 1v. Complete description of equipment services performed including upgrades, updates and software changes made. v. Complete list of all materials, parts, or assemblies installed or replaced including serial numbers of parts where applicable. v1. A statement of the complaint that initiated the service call. Total standard labor hours, total overtime/premium hours, and total travel hours expended during the repair. NMVAHCS shall not provide tools, (test) equipment, service manuals, or service diagnostic software to the contractor. Contractor shall obtain, have on file, and make available to its FSE all operational and technical documentation (such as operational and service manuals, schematics, and parts list) which are necessary to meet the performance requirements of this contract. The location and listing of the service manuals, by name and/or the manuals themselves shall be provided to the CO upon request. 1x. Contractor shall provide the COTR or Biomedical Engineering two (2) copies of all documentation that pertains to any hardware, software, or firmware upgrade or repair. Contractor shall provide to the COTR or Biomedical Engineering OEM Service Bulletins for the equipment covered under this contract. All documentation and service bulletins, etc. listed in this paragraph B shall be provided within 30 days of award in either hard or electronic format. Any future service bulletins generated during the contract period shall be provided to the COTR with 30 days of receipt; hard or electronic format is acceptable. DOCUMENTATION/REPORTS: 1. The documentation (ESR) shall include detailed descriptions of the scheduled and unscheduled maintenance (i.e., Emergency repairs) procedures performed, including replaced parts, and prices (for outside normal working hour services) required to maintain the equipment in accordance with Section III, Conformance Standards, and the manufacturer's specifications. Such documentation shall meet the guidelines as set forth in the Conformance Standards section. The ESR will consist of a separate PM report for the item(s) covered under the "specific" contract. Grouping different equipment from different contracts on "one" ESR is prohibited. In addition, each ESR must, at a minimum, document the following data legibly and in complete detail: 11. Name of contractor and contract number. 111. Name of FSE who performed services. 1v. Contractor service ESR number/log number. v. Date, time (starting and ending), equipment downtime, and hours on-site for service call. v1. VA purchase order number(s) covering the call if outside normal working hours. Description of problem reported by COTR/user (if applicable). Identification of equipment to be serviced: Inv. ID number, manufacturer's name, device name, model number, serial number, and any other manufacturer's identification numbers. 1x. Itemized Description of Service Performed (including, if applicable, Costs associated with after normal working hour services) including: Labor and Travel, Parts (with part numbers) and Materials and Circuit Location of problem/corrective action. Total Cost to be billed (if applicable -- i.e., part(s) not covered or service rendered after normal hours of coverage). x1. Signatures: FSE performing services described. 2. Authorized VA Employee who witnessed service described. NOTE: Any additional charges claimed must be approved by the COTR or their designee before service is completed. REPORTING REQUIRED SERVICES BEYOND THE CONTRACT SCOPE: The Contractor shall immediately, but no later than 24 (twenty-four) consecutive hours after discovery, notify the CO and COTR (in writing) of the existence or the development of any defects in, or repairs required, to the scheduled equipment which the Contractor considers he/she is not responsible for under the terms of the contract. The Contractor shall furnish the CO and COTR with a written estimate of the cost to make necessary repairs. Hours of Coverage: Normal hours of coverage are Monday through Friday from 7:30 am to 4:00 pm Arizona time (no daylight savings) and 8:00 am to 4:00 pm during WHEN hours. All service/repairs will be performed during normal hours of coverage unless requested or approved by the COTR or his/her designee. NOTE: Hardware/software update/upgrade installations will be scheduled and performed during normal hours of coverage at no additional charge to the government. Federal Holidays observed by NAVAHCS are: New Years' Day Martin Luther King Day Presidents' Day Memorial Day Independence Day Labor Day Columbus Day Veterans' Day Thanksgiving Day Christmas Day IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: A. The Contractor's FSEs shall wear visible identification issued by NMVAHCS Police Section at all times while on the premises of NMVAHCS. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). The NMVAHCS shall not provide tools, test equipment, service manuals, or service diagnostic software to the contractor. Contractor shall provide all tools, test equipment, service manuals, and/or service diagnostic software. Contractor shall obtain, have on file, and make available to its FSE all operational and technical documentation (such as operational and service manuals, schematics, diagnostic software, and parts lists) which are necessary to meet the performance requirements of this contract. The location and listing of the service manuals, by name and/or the manuals themselves shall be provided to the CO upon request. Contractor shall provide the NMVAHCS Biomedical Engineering Radiology Section copies of all documentation that pertains to any hardware, software, or firmware upgrade or repair.
