Description
COMBINED SYNOPSIS/SOLICIATION 36C25718Q9733 This is a combined synopsis/solicitation for commercial services in accordance with the FAR Subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested. You may email Ross Futch at ross.futch@va.gov for a copy of SF1149; if unable to download copy from FBO. Otherwise a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ 36C25718Q9733. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). Network Contracting Office 17-SAO WEST intends to award a firm-fixed price contract with base year and 4 option years for annual compliance inspection of patient isolations rooms and medical gas distribution systems located at the VA Texas Valley Coastal Bend Health Care System. The North American Industrial Classification System (NAICS) code for this procurement is 811219 with a business size standard of 20.5 Million. This procurement is restricted to SDVOSB offerors only to submit proposals. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. Offerors must be registered and verified in VetBiz (VA Vendor Information Pages). The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price technically acceptable and meets the requirements of the SOW. To be rated technically acceptable all personnel presented to provide inspection / conduct testing for this requirement must have NITC Medical Gas Inspector 6020 Certification. Offerors must provide proof of NITC Medical Gas Inspector 6020 Certification with quote submission. Past performance will be evaluated by checking past performance systems. Those offerors found with substantiated unacceptable past performance may be found to be unacceptable and not considered for award. The offerors quote found to be technically acceptable will be evaluated by price as lowest price, technically acceptable. Proposals are due by 4:00 PM Central Standard Time on August 9, 2018. This combined solicitation/synopsis is for the purchase of the following commercial services: SCHEDULE OF SUPPLIES/SERVICES SCOPE OF WORK STATEMENT OF WORK VA Texas Valley Coastal Bend Health Care System Patient Isolation Rooms / Medical Gas Distribution Evaluation / Certification Project GENERAL INFORMATION 1. Title of Project: VA Texas Valley Coastal Bend Health Care Systems (VATVCBHCS) Patient Isolations and Medical Gas Distribution system inspection / certification, Nitrous Oxide Survey, Halogenated Anesthetic Survey and Glutaraldhyde Survey. Scope of Work: Annual Compliance Inspection of Patient Isolation Rooms and Medical Gas Distribution Systems and Industrial Hygiene Assessment to measure occupational exposure to waste anesthetic gases (Nitrous Oxide & Halogenated anesthetics) and Glutaraldehyde. Contractor will visit all VATVCBHCS facilities at the addresses listed below for an on-site performance validation and supporting documentation so that the staff and patients are afforded proper protection, have a certified location for airborne disease containment and have required medical gases available at recommended amounts. For the industrial hygiene assessment, passive dosimeter monitoring for stressors mentioned above may be conducted. Upon conclusion of evaluation, the Contractor must provide a Service Ticket on-site along with a Certificate of Inspection & Compliance of every site-specific service visit. This Certificate of Inspection/Compliance will be maintained on-site so that audit practices may be satisfied. Contractor shall begin the work required under this Statement of Work within 10 calendar days or less from date of award, unless otherwise directed by the Contracting Officer. Contractor should make arrangements to visit each facility listed below during regular business hours, which is Monday through Friday 8 AM to 4:30 PM (excluding Federal Holidays) for each location: Corpus Christi Outpatient Clinic (CCOPC) 5283 Old Brownsville Rd. Enterprise Parkway Corpus Christi, TX. 78405 ~ Inspections to be performed: Isolation Room Ventilation Test and Every Outlet Harlingen Ambulatory Surgical Center Hospital (HCC) 2601 Veterans Blvd. Harlingen, TX. 78550 ~ Inspections to be performed: Isolation Room Ventilation Test, Waste Gas (O.R.) Ventilation Test and Every Outlet testing. Harlingen Outpatient Clinic (HOPC) 2106 Treasure Hills Blvd. Harlingen, TX. 78550 ~ Inspections to be performed: Isolation Room Ventilation Test and Every Outlet Testing Laredo Outpatient Clinic (LOPC) 4602 N. Bartlett Ave. Laredo, TX. 78041 ~ Inspections to be performed: Isolation Room Ventilation Test and Every Outlet Testing McAllen Outpatient Clinic (MOPC) 901 E. Hackberry Ave. McAllen, TX. 78503 ~ Inspections to be performed: Isolation Room Ventilation Test and Every Outlet Testing Ventilation Testing # s CCOPC 1 CCPact 0 CCSpecialty 4 HCC 51 HOPC 2 LOPC 2 MOPC 3 Medical Gas (Every Outlet) # s CCOPC 0 CCPact 0 CCSpeciality 16 HCC 68 HOPC 13 LOPC 3 MOPC - 4 NOTE: Access to the VA VPN IS NOT AUTHORIZED nor is it part of this SOW. 2. Background: The resultant impact of this project will facilitate the performance validation of Patient Isolation and Medical Gas Distribution Systems in all VATVCBHCS Health Care Facilities so equipped with the aforementioned fixed capital assets. 3. Performance Period: The contractor shall begin the work required under this SOW within 10 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Inspection date/time will be Monday through Friday 8 AM to 4:30 PM (excluding Federal Holidays). 4. Type of Contract: Firm Fixed Price B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. 2. Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within 15 calendar days from the date of receipt. The contractor shall have 15 calendar days to deliver the final deliverable from date of receipt of the Government s comments. 3. Contractor must provide proof of US Citizenship and Drug Screening of all employees who will potentially visit any VA Texas Valley Coastal Bend Health Care Facilities listed in the Scope of Work section. D. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. E. REPORTING REQUIREMENTS The progress report shall cover all work completed during the preceding month and shall present the work to be accomplished during the subsequent month. This report shall also identify any problems that arose, along with a statement explaining how the problem was resolved. This report shall also identify any problems that have arisen but have not been completely resolved, with an explanation. F. GOVERNMENT RESPONSIBILITIES Access to addresses Monday through Friday 8 AM to 4:30 PM (excluding Federal Holidays), as well as VA Facilities Management Personnel to serve as escorts. G. CONTRACTOR EXPERIENCE REQUIREMENTS If a key person becomes unavailable to complete the SOW, the proposed Substitutions of key personnel shall be made only as approved directed by the Contracting Officer and the COR. The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee's performance is unsatisfactory. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. The contractor must inform the VHA COR, and Project Manager / Task Manager when personnel are removed from the contract for any reason. If a key person becomes unavailable to complete the SOW, proposed Substitutions of key personnel shall be made only if approved by the COR. The contractor shall submit a resume of qualifications to the COR for all direct employees proposed for the project. All Contractor employees will be approved by the COR prior to bringing on duty. H. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. Information Systems Officer, Information Protection: The contractor will not have access to VA Desktop computers nor will they have access to online resources belonging to the government while conducting services. Privacy Officer: The contractor will not have access to Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA. Records Manager: There will be records created, and maintained with this contract in both paper and electronic format.
