Description
SF-18 Clauses Statement Of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 and 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issues. You are invited to submit a quote in response to the U.S. Department of Transportation, Pipeline and Hazardous Materials Safety Administration (PHMSA) Request for Quotation (RFQ) #693JK318Q0019. Please see the attached Statement of Work (SOW) for the description of the services to be purchased under this RFQ. Also, please read this document in its entirety as it contains pertinent information (quote contents, evaluation criteria, due dates, etc.) regarding this RFQ. This solicitation documents and incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC) FAC 2005-97, Effective July 16, 2018. 1. ELIGIBILTY, AWARD TYPE AND FAR APPLICATION: · Eligibility: o 100% Small Business set-aside. o Applicable NAICS code: 334310 – Audio and Video Equipment Manufacturing. o Business Size Standard: $15.5 Million · Award Type: Firm Fixed-Price Purchase Order under FAR Par 12 and 13.5. · FAR Application: Part 12 and 13.5 Award will be made to the responsible quote that represents the best value to the Government, using a tradeoff process based on the Evaluation Criteria in Section 3 below. The following documents are attached to this solicitation: Attachment 1 – Statement of Work (SOW) Attachment 2 - FAR, TAR, and PHMSA Clauses and Special Requirements 2. Request for Quotation (RFQ) Objective/Requirements: The Pipeline and Hazardous Materials Safety Administration (PHMSA), an agency within the Department of Transportation (DOT), is responsible for regulating the transportation of hazardous materials by highway, rail, air, water, and pipeline and ensuring that the Nation's 2.4 million miles of natural gas and hazardous liquid pipelines are safe, reliable, and environmentally sound. In order to fulfill its mission of promoting safety and conducting accurate inspections PHMSA requires a well-trained workforce. PHMSA inspectors are routinely trained at PHMSA’s Training and Qualifications Training Facility (TQ Facility). Through this requirement PHMSA seeks to ensure that the TQ Facility is equipped with the tools and furnishings necessary to give PHMSA inspectors an effective learning experience. The Pipeline and Hazardous Materials Safety Administration (PHMSA), Office of Pipeline Safety (OPS), seeks a contractor who will provide and install audio and visual equipment upgrades for PHMSA’s Twister and Chisholm Trail Classrooms located at the US DOT/PHMSA Training and Qualifications Training Center (TQ Center) located at 3700 S. MacArthur Blvd., Suite B, Oklahoma City, Oklahoma, 73169. The Statement of Work (SOW) (Attachment 1) contains the description of the services to be purchased under this RFQ. 3. Required Quotation Content The quotation must include the following: a. A Technical Capability Section This section must address the following subjects: i. Plan/approach for accomplishing the SOW; ii. Qualifications of the company to perform the SOW; iii. Resources to fulfill the requirements in the SOW; and iv. Resumes of proposed key personnel who will accomplish the tasks in the SOW. Quotations may provide no more than eight resumes; v. Time needed to install equipment after bid acceptance, and vi. Equipment requested b. A Past Performance Section This section should describe the contractor’s relevant experience in providing the required services. Quoters must provide four references (customers) who received similar services from the contractor within the past five years. For each reference, quoters should provide: organization name, contract number, contact name, e-mail address, telephone number, a summary of specific services performed, and dates of performance. c. A Price Section Quoters must submit pricing that is equal to open market pricing. Price discounts are highly encouraged. Prices must not be mentioned by the quoter in the Technical Capability and Past Performance sections of the quotation package. d. Additional Required Content Quoters must also provide the following additional information in their quotation package: i. Tax Identification Number (TIN); ii. Data Universal Numbering System (DUNS) number; iii. The actual SF-18 form; and iv. One copy of all RFQ amendments issued (if any) signed by an authorized official of the quoting company. e. Page Limitations The entire quotation package, including cover page and table of contents (if included) must be no longer than 35 pages total in length, 8 ½ by 11-inch paper using font size no smaller than eleven (11) font. Resumes will count toward the 35-page limitation, but the SF-18 form and RFQ amendments (if any) will not count toward the 35-page limitation. If the quoter exceeds this page limit then the quotation package will not be determined to be non-responsive; however, only the first 35 pages of that package will be reviewed. For purposes of this page limitation the Government will count quotation package pages in the following order: (1) Price Section, (2) Technical Capability Section, then (3) Past Performance Section. 4. Evaluation of Quotations: The award will be made to the responsible quoter whose quote conforms to the RFQ and offers the Best Value to the Government, in accordance with FAR part 13 (Simplified Acquisition Procedures) as a small business set-aside. The Government will use tradeoff analysis when making the best value determination and award, based on the following three factors: a. Evaluation Factor 1 - Technical Capability: The Government will evaluate the strengths and weaknesses of the below items as they relate to the requirements outlined in the SOW: 1. Plan/approach for accomplishing the SOW; 2. Qualifications of the company to perform the SOW, including but not limited to, the quoter’s: a. Expertise in working with and installing audio visual equipment, b. Resources to fulfill all requirements in the SOW, specifically the video projectors’ and c. Availability to install and then service the projectors and additional equipment in a timely manner, no greater than 24 hours from the time of service request. b. Evaluation Factor 2 –Past Performance: The PHMSA Acquisition Services Division (PHF-20) will evaluate the Quoter’s past performance as follows: · The contractor’s experience providing services similar to those required in the SOW. · References (customers) who received similar services from the contractor within the past five years. · The evaluation team may consider, but not be limited to, the quality of work, customer service, compliance with performance schedules, period of performance length, contract value, and contractor’s cost control on those contracts. · PHMSA, at its discretion, may obtain information regarding the quoter from the Past Performance Information Retrieval System (PPIRS) and the Federal Awardee Performance and Integrity Information System (FAPIIS) and other Government data sources. **NOTE** Prior to issuing any award, the Government will perform a search of Active Exclusions in the System for Award Management (SAM) to ensure that the prospective awardee is not on the list of parties excluded from receiving federal contracts and sub-contracts. c. Evaluation Factor 3 - Price: The PHMSA Acquisition Services Division (PHF-20) will evaluate the quoter’s price. Price discounts are highly encouraged. 5. Evaluation Order of Importance Evaluation Factor 1 (Technical Capability) is more important than Evaluation Factor 2 (Past Performance). Evaluation Factors 1 and 2, when combined, are significantly more important than Evaluation Factor 3 (Price). 6. Incorporation of Contract Terms Any resultant order from this RFQ will incorporate the terms and conditions stated in the SOW and the clauses and special requirements specified in Attachment 2: FAR, TAR, and PHMSA Clauses and Special Requirements, are applicable to the order. 7. Contract/Order Type and Award: The Government intends to issue a fixed price purchase order under the provisions of FAR Parts 12 and 13.5 from this RFQ to a small business. 8. RFQ QUESTIONS: Questions regarding the RFQ must be received via email by Mr. Stephen Jones (Stephen.jones.ctr@dot.gov) with a cc: to Mr. Fazal Mirza (fazal.mirza@dot.gov) on or before August 7, 2018 at 11:00 am, Eastern Daylight Time. Emails must reference RFQ #693JK318Q0019 and the company name in the subject line. PHMSA will consolidate all questions and issue response(s) to questions via email no later than August 14, 2018 at 2:30 pm, Eastern Daylight Time. Do not contact any other PHMSA personnel during the RFQ (pre-award) phase. 9. SUBMISSION OF QUOTES: Quotes must be received via email in PDF format by Mr. Stephen Jones at (Stephen.jones.ctr@dot.gov) with a cc: to Mr. Fazal Mirza (fazal.mirza@dot.gov) no later than August 21, 2018 at 11:00am, Eastern Daylight Time. *Quoters please note, the DOT email system will not accept attachments larger than 10MB in size. Attachments exceeding 10MB in size will be rejected by the DOT mail server. Please ensure your quote conforms to the DOT email size restrictions. The burden of providing and ensuring timely receipt is on the quoter.
