- Notice of Intent to Sole Source: Amendment 36C25918P4521 The Department of Veterans SAO West, Network Contracting Office 19, located at 6162 South Willow Drive Suite 300, Greenwood Village, CO 80111 is hereby, amending the intent to enter into a sole source purchase order agreement authorized by FAR 13.106-1(b), Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. Under the authority of FAR 13.106-1(b), the Department of Veterans Affairs intends to award a sole source purchase order to the large business, Cook Medical LLC, 1025 Acuff Road, Bloomington, IN, in accordance with FAR provision, 52.212-2(b), Evaluation Commercial Items. The associated North America Classification Systems (NAICS) Code is 339112 and the Business Size Standard is 1,000 employees. The contractor shall provide the George E. Wahlen VA Medical Center in Salt Lake City, Utah with Cook Medical Ballon Catheters to be used with the current Zilver PTX drug-eluting peripheral stent. This notice of intent is not a request for competitive proposals or quotes. Interested parties may identify their interest and capability to respond to the requirement. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the U.S. Government. Interested persons may identify their interests and capability to respond to this requirement no later than 4:00 PM Mountain Time, Monday, July 30, 2018. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government in future procurements. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to meet the subject requirement. This information shall be provided at no cost or obligation to the Government.
