Description
SAN-I-PAK PREVENTIVE MAINTENANCE STATEMENT OF WORK DESCRIPTIONS/SPECIFICATIONS/ORK STATEMENT Scope: The services to be provided by the contractor under this agreement shall consist of furnishing all parts, labor and transportation to provide preventive maintenance and repair SAN-I-PAK MARK VI-N autoclave sterilizer/compactor. VAMC Northport is seeking one monthly service call visit and 24-hour response by a certified technician for any mechanical problems that arise between monthly service visits. Background: VAMC Northport has a San-I-Pak Mark VI-N autoclave machine located at the loading dock of the main hospital-Building 200. This autoclave machine has an insulated pressure chamber in which saturated steam is used to elevate the temperature to decontaminate infectious waste. This machine allows for VAMC Northport to treat our red bag waste on-site rather than having it removed by a licensed medical waste contractor. Period of Agreement: Unless otherwise specified, this contract will be from October 1, 2018 through September 30, 2019 and four (4) option years. Hours of Coverage: Normal hours of coverage are Monday through Friday from 8:00 AM to 4:30 PM, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by the Contracting Officer (CO) or Contracting Officer Representative (COR). Work performed outside the normal hours of coverage at the request of the Contracting Officer or Contracting Officer Representative will be billed at the contractor s normal hourly rate and normal hourly travel rate (if applicable). Work performed outside the normal hours of coverage at the request of the contractor will be considered service during normal hours of coverage. Federal Holidays observed by VAMC Northport are: New Year s Day Labor Day Martin Luther King Day Columbus Day President s Day Veteran s Day Memorial Day Thanksgiving Day Independence Day Christmas Day Services to be Provided: Telephone Assistance: Telephone assistance will be provided twenty-four (24) hours per day, seven (7) days per week. Parts: The contractor shall furnish and replace parts as required on an exchange or new parts basis. The contractor has ready access to unique and/or high mortality replacement parts. All parts supplied shall be compatible with existing machine. The contract shall include all parts with no exceptions. The contractor shall use new or re-built parts. Used parts, shall not be installed without approval by the COR or EMS Chief. Repair: Inadequate or improper repairs performed by the contractor will be correctly repaired at a time convenient to the VA at no additional expense to the VA. Preventive Maintenance: Preventive maintenance inspections are required monthly. Preventative maintenance inspections will be performed in a manner that clearly demonstrates the equipment meets or exceeds all manufacturers technical performance specifications. Maintenance will include, but not restricted to: 1. Note general condition of unit 2. Adjust pressure regulator as required   3. Check jacket temperature (230 degrees) and jacket pressure (46 PSI)   4. Check steam pressure (58-60 PSI) 5. Check sterilizer chamber and scrape out any plastic residue or debris  6. Check liner screw shaft.   Remove residue and debris  7. Open sterilizer load door; clean gasket seal area; replace as necessary    8. Check recorder paper roll and replace as required.    9. Observe a sterilizing cycle noting:        a. Vacuum reaching at least twenty (20) inches.          b. Chamber steam pressure reaching 36 PSI          c. Sterilizer temperature of at least 281 degrees.      Warning:  Ensure main disconnect in the off position, the steam supply is turned off and the emergency stop button is pushed before proceeding!  Warning:  Steam piping and chamber are hot! Make sure protective clothing is worn around these items!    Check hydraulic oil level.   Oil should be visible on site glass and ¾ full  Check all hydraulic hoses and fittings fo leakage, tighten as necessary    Visually check all hydraulic and electrical components    Remove the two (2) long stainless access panels located on either side of the machine. Clean the entire length of the wheel tracks and apply a film of grease. Re-install access panels    Remove lower rear access panel. Locate grease fittings for the compactor wheels located on the back of the compactor ram    Inject grease into the grease fitting for each wheel    Warning:  Steam pipes are hot! While all access panels are removed, thoroughly clean all debris from in and around hydraulic motor and under machine. It is extremely important that all debris be removed as it presents a fire hazard and can interfere with the proper operation of the unit. After debris has been removed, wash down the entire area under the machine to remove any hydraulic oil that may have leaked.    Warning/Caution:  Turn off steam supply and allow sterilizer equipment to cool to the touch!    Rinse sterilizer chamber with tap water only as required. Never use bleach or chlorine base cleaners/disinfectants    Reviewing operating system diagnostics (hardware and software) to ensure that the system is operating within its specifications Calibration, lubrication, adjusting, and testing of the equipment according to machine specifications Inspecting electrical wiring for wear and fraying Replacing components as part of the recommended preventative maintenance procedures It is the sole responsibility of the contractor to perform all preventative maintenance inspections. The VA will not assume the responsibility of reminding the contractor that a preventative maintenance inspection is due to be performed. Preventative maintenance inspections will be performed on a weekday at a time set once the contract is awarded. The equipment user, COR and contractor must all agree to the recurring time set for the preventative maintenance inspections. Note: To ensure proper equipment history documentation, the contractor will forward the preventative maintenance schedule and procedures to Environmental Management Service once it is set. The VA Medical Center shall provide the contractor with full, free and timely access to the equipment to form this service. The contractor will provide all necessary personal protective equipment for their service representatives to use while performing any service on this equipment. The VA will not provide PPE for the contractor s employee. Additionally, if the device is subject to OSHA lock-out/Tag-out regulations, the contractor will provide the necessary lock-out/tag out equipment to satisfy the OSHA regulations. The VA will not provide lock-out/tag-out devices to the contractor. Scheduling Arrangements: Routine service calls are ones, which, in the estimation of the VA, are necessary to diagnose and repair equipment problems, but can be performed within normal working hours. Response time of the contractor for routine service calls will be no more than twenty-four (24) hours after being officially notified. Routine service calls outside normal working hours will result in the contractor responding no more than twenty-four (24) hours commencing the next available working day. Emergency service calls are one which, in the estimation of the VA, requires the immediate response of the contractor. Response time of the contractor for emergency service calls made during normal working hours will be four (4) hours after being officially notified. Emergency service calls made outside normal working hours will result in a response time of the contractor within four (4) hours commencing the next available working day. The term Response time of the contractor indicates the physical presence of a fully qualified field engineer arriving on station within the specified time frame. Official notification for the need of service will be considered given at the time a message is left with the mechanism the contractor has in place to inform employees of the VA s needs. Response time will commence once the notification is given. Special Instructions: Instructional meeting: On the first visit to the site during the contract period, there will be a meeting of COR, EMS Manager and contractor to discuss policies and expectations. Contractor s representative will report to Environmental Management Service, Building 6, Room 106, prior to performance of service; this check-in is mandatory. In the event that the contractor will reports after normal working hours (emergency service calls only), the contractor will report to the supervising official of the equipment to be serviced. Documentation: Contractor will provide individual written service reports which describe services performed on the equipment. The following information must be provided in each service report: -Date of service -Summary of problem and action taken -parts replaced -Labor hours -Travel time -Equipment serviced (description, equipment ID#, serial number, etc.) -Purchase order number (if necessary) Note: This report must be approved and signed by Environmental Management Service personnel. A copy of each report will be left with EMS. In the event that EMS is not available to receive the service report (outside normal working hours), the service report will be left with the supervising official of the equipment that was serviced; the supervising official will forward the service report to EMS the next available working day. Qualifications: Bidders shall be engaged full-time in the maintenance and repair of similar equipment as the one under the proposed contract. Contractor shall have access to complete service manuals and a complete parts inventory. Vendor Competency Verification: This documentation shall provide proof that the service representatives have completed appropriate approved training on the equipment being serviced. This documentation shall include dated training certificates and course topics or outlines if available. Terms and Conditions: User Responsibility: Contractor will not be held responsible under the terms of this contract for repairs to equipment necessitated by user abuse and/or misuse of equipment. Exclusions: Service does not include electrical work external to the equipment.
