Description
VETERANS AFFAIRS MEDICAL CENTER, ERIE, PA GENERAL INFORMATION FOLLOWED BY A TENTATIVE STATEMENT OF WORK NOTICE: This is not a solicitation but rather a Request for Information (RFI) (Sources Sought) to determine capability of potential sources and is for information and planning purposes only. Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify capable firms to provide height-adjustable reclining sit bath for a newly constructed Community Living Center. The Erie VA Medical Center. The units must be rated for commercial use to withstand heavy use be within hospital specification standards. Responses must be submitted by COB August 2nd 2018. Submit responses to the information requested above via email to rashaun.thomas@va.gov. All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this source sought notice for market research purposes, and shall include as part of their response a brief capability statement that covers the information in the following tentative statement of work. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. Accepting information on selecting more appropriate NAICS and PSC codes. NAICS code 339113 and PSC 6515 was used for Contract # 47QSWA18D002W / Solicitation #: 7FCM-C4-030073-B on GSA E-Buy. NAICS code 339113 was found to have been used by the previous vendor as captured by Assertions tab in the System for Award Management (SAM). Suggested NAICS code: 339113 Suggested PSC code: 6515 If there are more appropriate NAICS and/or PSC codes than what is suggested, please include as part of your full sources sought response. Tentative/Sample Statement of Work: Background: The use of a reclining sit bath/spa tub will provide residents with relaxation and promote circulation and exfoliation. It will also allow for range of motion with less resistance and alleviate pain while the reclining feature offers fuller immersion in the warm waters. Salient Features: - Reclining function - Height adjustable for ergonomics - Must have air spa system which can be operated using a control panel and a handheld control unit - Must have a built-in disinfection system - Must have a thermometer display - Must have a hand-held shower - Width shall not exceed 29 - Length shall not exceed 78 in reclined position and 73 in upright position - Must have a side entry door - Must possess a maximum load capacity (resident + water) of at least 660 lbs. - Must possess a maximum resident/patient weight of at least 460 lbs. - Must possess a minimum transfer height of at least 23 at lowest position - Must possess a maximum transfer height of at least 27 at highest position - Must have a built-in battery back-up system Two (2) shoulder wrap Installation: Vendor shall unpack and setup equipment which will be comprehensive to allow immediate use upon completion. This will include testing the tub and verifying functionality is met to the satisfaction of the government. Training: The vendor shall provide comprehensive, on-site training within 60 days of installation/setup.
