Dental Unit Waterline Culture Testing STATEMENT OF WORK (SOW) 1. Contract Title. VA DENTAL CLINIC SAMPLE LOCATIONS FOR COLLECTION OF WATER FOR ANALYSIS USING ANALYTICAL METHOD 9215 FOR HETEROTROPHIC PLATE COUNT 2. Background. The Environmental Protection Agency (EPA), American Public Health Association and the American Water Works Association have defined the standard for US potable water not to exceed 500 colony forming units (cfu) per milliliter of water. The Centers for Disease Control and Prevention (CDC) and the American Dental Association (ADA) have recommended dental unit water line (DUWL) deliver water should meet the U.S. potable water standard not to exceed 500 cfu/ml of water. Dental services are required to routinely test water coming out of the DUWLs to meet this standard and have an action plan to meet this standard which addresses any DUWL that falls out of compliance. Waterline testing is preferred to be accomplished by a laboratory certified (EPA or state) to perform environmental testing. VA Microbiology Laboratories perform clinical tests, and have not been approved for environmental testing. 3. Scope. The Northport VA Medical Center Dental Service is required to test its Dental waterlines every quarter. Additional testing may be required for new equipment or Dental suite and in the event of new equipment or unacceptable testing where repeat testing necessary. The contractor will use a New State Certified water testing company with experience using Water Analysis Analytical Method 9215 for testing water lines and devices as described in the Statement of Work each quarter four times a year for each location. The dental areas are located at Northport VA Medical Center. Test each line separately; remove all hand pieces and any other equipment reprocessed by SPS before drawing samples from the lines.   Air-water syringe tips should be removed.   The air-water syringe itself, if not changed between patients should remain on the line while drawing the sample. 40 TO 50 WATER SAMPLES WILL BE TAKEN AS PER THE ENCLOSED SAMPLE LAB REPORTS FOR EACH CAMPUS. Samples are to be collected from the wet/dry syringes, high speed hand piece, cuspidor, water reservoir bottle, or tap water and at one ultrasonic cleaner. Before collection of samples, water is to be flushed for one minute at each source. 4. Specific Tasks. Task 1 Prepare/package/deliver required testing bottles and associated paperwork to the Northport VA Medical Center Dental Clinic when requested by dental personnel. Dental personnel will indicate the number of waterlines to be tested. Task 2 Pick-up testing bottles and associated paperwork from the Northport VA Medical Center Dental Clinic once requested by dental personnel and within 24 hours of water samples being obtained. Task 3 Perform testing for Bacterial - Heterotropic Plate Count (HPC) on water samples per standard operating procedure, CDC guidelines and VA Infection Control protocol. Task 4 Compile water testing results and submit final report (via email) to Northport VA Medical Center Dental Clinic. Task 5 - The Dental Clinic will forward the final report to the Northport VA Medical Center Infection Control Committee. 5. Performance Monitoring (if applicable). The testing Laboratory must be certified by testing Lab State or other accreditation body under the provisions set forth in Section 502 of the New York State Public Health Law. Certification documentation to be provided annually. 6. Security Requirements. The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall require intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required. The C&A requirements do not apply and a Security Accreditation Package is not required. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Not applicable 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. Not applicable b. Identification of Potential Conflicts of Interest (COI). Not applicable c. Identification of Non-Disclosure Requirements. Not applicable d. Packaging, Packing and Shipping Instructions. Testing Lab will provide water testing bottles in a container safe for transport. Dental personnel will use this same container to package testing bottles once samples have been obtained. If additional testing is required Testing Lab will provide testing bottles and shipping materials. Dental personnel will indicate the number of waterlines to be tested. e. Inspection and Acceptance Criteria. The Dental Service Chief will appoint a Staff Dentist to manage the waterline testing program. The testing Lab will submit the final report to the Staff Dentist who will in turn review the results and forward a copy to the Northport VA Medical Center Infection Control Committee. 9. Risk Control Not applicable 10. Place of Performance. Water samples are obtained by Dental Service at Northport VA Medical Center. Testing will be performed at certified Environmental Water Testing Lab defined in the contract. 11. Period of Performance. The period of performance for the base period is expected to run from October 1, 2018 to September 30, 2019 with four option years to be exercised at the discretion of the government. 12. Delivery Schedule. The contractor will email the results of the water testing within 24 hours after testing has been completed to the COR at the Northport VA Medical Center Dental Clinic. The Dental Clinic will forward the final report to the Northport VA Medical Center Infection Control Committee. Contact dental equipment manufacturer and waterline treatment manufacturer for recommendations on options, such as shocking or line replacement. Drawing samples, transportation to lab and report are all included in the FFP at no additional cost to the US Government.
