INTRODUCTION The Veterans Health Administration (VHA), Program Contracting Activity Central (PCAC) anticipates soliciting quotations and subsequently awarding a Firm-Fixed-Price contract for Phlebotomy Workstations for the VA Northern California Health Care System (VANCHCS). Acquisition of these items is part of the initial outfitting and activation of the new Chico VA Clinic in Chico, CA. DESCRIPTION A contract is needed for the Chico VA Clinic for below listed brand name or EQUAL equipment. SALIENT CHARACTERISTICS/MANUFACTURERS CUTSHEETS The contractor will be required to include manufacturer cut-sheets/specifications for or equal items quoted. CLIN 0001 Workstation, Lab, Phlebotomy, End Unit, Left Side (E0570A) Qty - 1 Salient Characteristics: Ergonomic modular phlebotomy station oriented to the left side of the patient Provides storage at or above waist to prevent bending reaching or turning Can accommodate wheelchairs Phlebotomist side: Unit includes glove box holder (2) Slanted shelving designed to hold blood collection tubes Cavi-wipe holder Secured trash can Sharps holder Patient side Coat hook Vertical and horizontal grab bars Small shelf for personal belongings Approximate Dimensions (+/- 2 ): 14 9/16"wide x 42 1/2" deep x 66" high Final finishes and colors to be coordinated/approved by VA interior design prior to purchase CLIN 0002 Workstation, Lab, Phlebotomy, End Unit, Right Side (E0570B) Qty - 1 Salient Characteristics: Ergonomic modular phlebotomy station oriented to the right side of the patient. Can accommodate wheelchairs Phlebotomist side: Unit includes glove box holder (2) slanted shelving designed to hold blood collection tubes Cavi-wipe holder Secured trash can Sharps holder Patient side: Coat hook Vertical and horizontal grab bars Small shelf for personal belongings Approximate Dimensions (+/- 2 ): 26 3/8" wide x 42 1/2" deep x 66" high Final finishes and colors to be coordinated/approved by VA interior design prior to purchase CLIN 0004 - Phlebotomy Table, Wheelchair Accessible (M1411A) Qty - 2 Salient Characteristics: Adjustable and mobile draw stand for providing phlebotomy services to wheelchair bound patients Stand consists of flat height adjustable non porous solid surface, locking casters, interlocking break system Work surface shall adjust vertically 27"-42" above floor Unit shall have interlocking casters Locking mechanism controlled by pedal foot controls Draw stand surface shall be non-porous, resistant to blood stains and cleanable with a 10% bleach solution Stand top has recessed sections to prevent items from falling off table Approximate Dimensions (+/- 2 ): 28" wide x 19" deep x 27"-42" high ADMINISTRATIVE The solicitation will be issued as a Request for Quote (RFQ) conducted in accordance with Federal Acquisition Regulation (FAR) Parts 12 and 13. The solicitation will be advertised as a total Small Business set-aside. The applicable NAICS code is 337127 with a size standard of 500 Employees.   The solicitation will be issued on or about August 1, 2018 for approximately seven (7) calendar days. Offerors will be required to submit their quotes electronically via email. Products that are purchased outside of our authorized distributor network on the gray market generate a number of concerns such as: counterfeit or adulterated devices, inadequate storage conditions, tracking issues in case of a recall, loss of warrantee, and most importantly patient safety.   In some instances unauthorized distributors procure products from an authorized distributor for resale. However, authorized distributors purchase products at list price, and the unauthorized distributors are reselling the product at a price point below list price, which raises questions about product sourcing. The solicitation will include the requirement that responders provide an authorized distributor letter that states the responder is an authorized dealer/distributor of the OEM for the items being procured such that all product warranties and service and/or preventative maintenance agreements transfer to the VA and will be honored by the OEM or guarantees that the OEM gives authorization to the responder to fulfill all warranty, service, and/or preventative maintenance obligations for the equipment. Vendors who are unable to supply proof of authorized dealer or distributorship will not be considered for award. This notice is not a request for competitive offers. Questions of a technical nature will not be answered as part of this notice; however, all general questions may be submitted in writing to the issuing office via email at jacquelyn.wise@va.gov AND michelle.rhodes@va.gov. Telephone calls will not be accepted. The VA reserves the right to use only Federal Business Opportunities (FBO), via the internet, to provide notification of any amendments to this notice and the subsequent issuance of the solicitation.
