- Notice of Intent to Sole Source: 36C25918P4538_1 The Department of Veterans SAO West, Network Contracting Office 19, located at 6162 South Willow Drive Suite 300, Greenwood Village, CO 80111 is hereby, amending the intent to enter into a sole source purchase order agreement authorized by FAR 13.106-1(b), Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. Under the authority of FAR 13.106-1(b), the Department of Veterans Affairs intends to award a base plus four (4) option year, sole source purchase order to the small business, Drive Safety, Inc., 426 White Pine Drive, Salt Lake City, UT, in accordance with FAR provision, 52.212-2(a)(i), Evaluation Commercial Items. The associated North America Classification Systems (NAICS) Code is 624310 and the Business Size Standard is $11 million. The contractor shall provide the George E. Whalen VA Medical Center in Salt Lake City, Utah with the CDS-250W (Clinical Driving Simulator) that is a compact, non-immersive driving simulation system optimized for use in clinical settings. It is designed as a clinical tool for occupational and physical therapy, as well as driver rehabilitation and driving performance measurement for ambulatory, wheelchair transfer and non-transfer clients. This notice of intent is not a request for competitive proposals or quotes. Interested parties may identify their interest and capability to respond to the requirement. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the U.S. Government. Interested persons may identify their interests and capability to respond to this requirement no later than 4:00 PM Mountain Standard Time, Monday, July 30th, 2018. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government in future procurements. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to meet the subject requirement. This information shall be provided at no cost or obligation to the Government.
