Sources Sought/Market Survey Notice This is a Sources Sought for availability of brand only CIRCUIT BREAKERS as Electrical commercial items under NAICS 335999, prepared in accordance with FAR Subpart 12.6, as supplemented for the Veterans Health Administration Ann Arbor Michigan. Responses should include the following information: (1) Business Name and Address, (2) Point of Contact Name and Phone Number and E-mail Address, (3) GSA/FSS Contract Number (if applicable), (4) DUNS, (5) SMALL or LARGE BUSINESS, and (6) Type of Small Business: Service Disabled Veteran Owned Small Business - or - Veteran Owned Small Business, etc. There may be a Set-Aside as a result of the information. The NAICS code may be waived under FAR 19.5. Items will be shipped to: VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. Responoses shall be submitted to Terry Van Allen, Contracting Specialist, at terry.vanallen@va.gov no-later- than Saturday, July 28, 2018 at 3:00 pm Eastern Time. Please respond to availability for brand, quantities, unit price, and total price for items below. Twelve (12) Square D molded case circuit breakers, model number LX36400G. Breakers are to be manufacturer Square D equipment, which has been reconditioned/re-certified breakers and tested and inspected with appropriate paper work and must be provided with a one-year warranty from supplier. One (1) Square D molded case circuit breaker, model number LX36600G. Breaker is to be manufacturer Square D equipment, which has been reconditioned/re-certified and tested and inspected with appropriate paper work and must be provided with a one-year warranty from supplier. Three (3) Cutler-Hammer Series C Industrial circuit breakers, catalog HKD3400F, style 1492D82G03. Breakers are to be new or re-certified with one-year warranty from supplier or manufacturer. Again, please provide availability for brand, quantities, unit price and total price for items above. The requested information is for supplies only and the VHA will perform installation.
