- Attachment 3 - Pricing Spreadsheet Attachment 2 - Clauses and Provisions Attachment 1 - Statement of Work (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number is 70Z02318QVXS50400 and is being issued as a Request for Quote (RFQ). The solicitation procedures of FAR 13.106 apply. The U.S. Coast Guard (USCG) anticipates awarding one Labor-Hour Purchase Order resulting from this RFQ. (iii) The solicitation document, and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-92. (iv) The North American Industrial Classification System (NAICS) code is 541990, All Other Professional, Scientific, and Technical Services. This procurement is a Total Small Business Set-Aside; submissions that do not meet the small business requirement will not be considered. (v) Offerors shall provide a quote, as requested in Attachment 2 - Clauses and Provisions, FAR 52.212-1 Instructions to Offerors (Jan 2017) - ADDENDUM. (vi) The USCG has a requirement for on-call consultation support to provide threat management and threat assessment services related to targeted violence, workplace violence, and insider threat cases for the Coast Guard Investigative Service (CGIS) Threat Management Unit (TMU). The USCG anticipates that this effort will consist of approximately 3-4 telephone calls per month, lasting approximately 30-45 minutes per call. These consultation services shall be provided in accordance with the requirements outlined in the Statement of Work (SOW) - Attachment 1. ( vii) The periods of performance includes a Base Period, from September 01, 2018 - August 31, 2019, and two (2) Option Periods: [Option Period 1] from September 01, 2019 - August 31, 2020 and [Option Period 2] from September 01, 2020 - August 31, 2021. Contract Type. Based on Lowest Price Technically Acceptable (LPTA), t he Government will award one Labor-Hour Purchase Order containing a Not-to-Exceed (NTE) contract line item number (CLIN) for the Base Period and NTE CLINs for Option Periods 1 and 2. Prices quoted by Offerors shall be all inclusive. Subsequent to award, invoices will be submitted for approval on a monthly basis, as services are rendered. (viii) The provision at FAR 52.212-1 applies to this procurement. Addendum to FAR 52.212-1 Instructions to Offerors - Commercial Items (Jan 2017) is in Attachment 2 - Clauses and Provisions. (ix) The provision at FAR 52.212-2, Evaluation-Commercial Items applies to this acquisition. Refer to Attachment 2 - Clauses and Provisions for evaluation factors. (x) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. See Attachment 2 - Clauses and Provisions. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition. See Attachment 2 - Clauses and Provisions. (xii) Additional clauses and provisions that apply to this acquisition are in Attachment 2 - Clauses and Provisions. The following attachments are included and is hereby incorporated into this solicitation: Attachment 1 - Statement of Work Attachment 2 - Clauses and Provisions Attachment 3 - Pricing Spreadsheet (xii) Questions about this combined synopsis/solicitation are due by 10:00 AM (Eastern) on Friday, August 03, 2018. Quotes are due via email no later than 2:00 PM (Eastern) on Thursday, August 9, 2018. Quotes shall be e-mailed to Mr. Rigor and Mrs. Baylor-Davis. There will be no exceptions to the time and date responses are due, unless determined otherwise by the Government. Any quote submitted after this time will not be considered. When responding, include the title "70Z02318QVXS50400 CGIS Threat Consultation Services" in the subject line of the e-mail. It is the Offeror's responsibility to ensure that quotes are submitted timely. No changes or additions will be permitted after the deadline, except as provided under discussions, if necessary. This RFQ does not represent authorization to award or proceed with the subject effort, nor a promise of future direction to proceed. Offerors are solely responsible for this RFQ's submission costs, even if this RFQ is cancelled. Offerors that do not submit all of the information required may not be considered for award.
