- Tax Exempt Letter Pricing Schedule Terms & Conditions Business Associate Agreement Quality Assurance Surveillance Plan Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal (RFP) and the reference number is RFP-18-PHX-07. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-98 and the Health and Human Services Acquisition Regulation (HHSAR). The Offeror(s) must quote on an all or none basis. Written quotes ARE required (oral offers will not be accepted). Submission of quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Item - Evaluation FAR 52.212-2. The Offeror shall complete 52.212-3 Offeror Representations and Certifications - Commercial Items, paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision. The basis for evaluation shall be on best value with tradeoffs. Offerors shall provide a three (3) to twelve (12) page Technical solution to the Performance Work Statement and a minimum of three (3) references who can validate recent and relevant past performance. Solicitation is issued using Simplified Acquisition Procedures in accordance with FAR 13.106-2 and FAR 13.106-3. Clauses can be obtained in full text at: http://farsite.hill.af.mil. Additional information required: CAGE Code, DUNS, Tax Identification Number, FOB Point, Discount Terms and Delivery Date. This procurement is unrestricted under associated NAICS Code 624110 (Child & Youth Services) with Size Standard of $11 Million. This is a requirement of obtaining Spiritual Counselor Services for Desert Visions located in Sacaton, Arizona. (The quote shall be valid through 30 September 2018) The Submission of Proposal is due as soon as possible but not later than 25 August 2018 at 3:00 PM Pacific Standard Time (PST). Basis for Award: The Phoenix Area Office of Acquisitions will issue a purchase order/contract to the vendor who is determination to have offered the best overall value to the Government. Special Notes: Best Value Determination. The technically acceptable, lowest priced quote will be determined to represent the best value. Technical acceptability will be determined by an evaluation of the quoted requirements that meet the technical criteria that is contained in the solicited item description. If the lowest priced submission does not meet the technical criteria described in the solicited item description, the government reserves the right to evaluate the next lowest priced submissions until it has determined a technically acceptable submission. The evaluation will stop at the point when the government determines an offeror to be technically acceptable with the lowest evaluated price, because that offer will represent the best value. Contingent upon a determination of contractor responsibility, award will be made to that offeror without further evaluation of other offers. Award will be made to the responsible vendor whose quote is the lowest evaluated price among technically acceptable quotes. Site Visit. Not Applicable. Notice to Offeror. The Government reserves the right to cancel this combined synopsis/solicitation request, either before or after suspense date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. Discussions. The Government intends to award a purchase order without discussions with respective vendors/offerors. The Government however, reserves the right to conduct discussions if deemed in its best interest.
