Description
Standard Form 18 Combined Synopsis/Solicitation Request for Quotation (RFQ): N6809618Q0021 Medical Waste Pick up and Disposal for USNH Guam and associated Facilities 1. This is a combined synopsis/solicitation for commercial items and/or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation ( N6809618Q0021 ) is being issued as a Request for Quotation. This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Regulations (FAR). All responsible sources may submit a proposal. 2. This procurement is set-aside to small businesses only. The North American Industry Classification System (NAICS) code is 562211 - Hazardous Waste Treatment and Disposal. Offerors must be registered in System for Award Management (SAM) at http://www.sam.gov, in order to be considered for award. 3. FAR Clauses 52.212-1 through 52.212-5 apply to this procurement. The Government contemplates an award of a Firm Fixed Priced purchase order contract resulting from this solicitation. 4. Description of Requirement: Pickup, process and disposal of regulated medical waste (RMW) generated by the U.S. Naval Hospital, Guam (USNH Guam) and its two off-site branch health clinics and NBG veterinary clinic. The contractor will pick up RMW at the RMW staging facility, Building no. 48-E2, three times per week (or as needed) and pick up RMW at NBG Branch Medical Clinic (Bldg. 6) and NBG Branch Dental Clinic (Bldg. 1), NBG Boller Veterinary Clinic, (Bldg. 104) when instructed or as needed. See Scope of work (SOW). This combined synopsis & solicitation notice is a request for competitive quotations. Therefore, all capability statements or quotations received prior to the RFQ close date will be considered. This requirement is for a Base Year and One (1) additional Option year at the Governments discretion. *Note: The contractor shall provide a copy of their Guam Environmental Protection Agency (GEPA) Solid Waste Processing Permit to the Environmental Protection Specialist, Facilities Management Department for filing. The contractor will comply with ALL Federal, Local and applicable laws regarding the disposal of RMW from NH Guam to the final disposal site for the duration of the contract (e.g. "Cradle to Grave"). 5. Place of Performance, Delivery Address: US Naval Hospital Guam Building 50 Farenholt Avenue Agana Heights, GU 96910 6. Basis for Award: The Government will award a single purchase order contract resulting from this RFQ to the responsible offeror whose offer represents the best value in terms of Technical requirement and lowest price technically acceptable. Technical requirement is defined as the ability of the contractor to meet the Scope of Work attached essential to meet the Government's Minimum need. Lowest price technically acceptable is defined when best value is expected to result from selection of the technically acceptable proposal with the lowest evaluated price. Technical requirements and lowest price technically acceptable when combined are significant evaluation factors that will be most advantageous to the Government. Contractors who do not meet both of the factors will not undergo a price evaluation. Note: As per 52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014) Technical evaluation - "At a minimum, the quote shall address the minimum technical specifications outlined in the solicitation with service specification literature demonstrating that the offeror is able to meet the Government's requirement." Responses to this notice should include: (1)Completed SF18 (Attached) (2) A written narrative of contractor's capability to provide the requirement and/or marketing Materials that identify the features of their particular enterprise; (3) Company size in relation to the applicable North American Industry Classification System code (NAICS) 562211 (4) Commercial and Government Entity (CAGE) Code; (5) The socioeconomic status of the company, i.e., small business concern, 8(a) business, HUB Zone small business, small disadvantaged business, woman-owned small business, veteran owned small business, or Service disabled veteran owned small business; 7. Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Item. Responsible sources must be registered and active in the System for Award Management (SAM) database, website accessed through http://www.acquisition.gov. 8. Responsible offerors must be able to utilize the Wide-Area-Work-Flow (WAWF) electronic invoicing as a method of payment. WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site. 9. Offers are due on Wednesday 08 AUG 2018 05:00 PM ChST. All questions/inquiries must be submitted via electronic mail (e-mail) not later than Friday 03 AUG 2018 3:00 PM ChST. Offerors who fail to complete and submit the requirements above may be considered non-responsive. This Combined Synopsis / solicitation shall not be construed as a commitment of any kind. 10. Special Notes: a. AMENDMENTS TO THE SOLICITATION: If this solicitation is amended, all terms and conditions that are not amended remain unchanged. Offeror shall acknowledge receipt of any amendment to this solicitation by the date and time specified in the amendment(s). Failure to acknowledge an amendment by the date and time specified may result in rejection of your proposal. b. EXCHANGES WITH OFFERORS AFTER RECEIPT OF PROPOSALS: The Government intends to evaluate proposals and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical stand-point; however, the Government reserves the right to con-duct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all proposals if such action is in the public interest; accept other than the lowest proposal, if the lowest proposal is not technically acceptable; and waive informalities and minor irregularities in offers received. c. PROVISIONS AND CLAUSES: Offerors must give special attention to the provisions/clauses in this solicitation. Applicable Clauses: It is the contractors' responsibility to be familiar with the applicable clauses and provisions.
