Description

COMBINED SYNOPSIS/SOLICITATION Sizewise Evolution Bed Frame for the Department of Veterans Affairs, Medical Center, Houston TX 77030. (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number:36C256-18-AP-0795 (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94, 2005-95 / 01-24-2018. (IV) This solicitation is being issued as a Service Disabled Veteran Owned Small Business Set-Aside/Veteran Owned Small Business. The associated NAICS code is 339112. The small business size standard is 1000 Employees. (V) This combined solicitation/synopsis is for purchase of the following commercial supply: Contractor shall furnish supplies for the Michael E. DeBakey VA Medical Center, 2002 Holcombe Boulevard Houston, TX 77030 (VI) Description of requirements is as follows: Contractor shall furnish all materials, parts, equipment, tools, transportation, and supervision to provide, on time delivery at the Michael E DeBakey Medical Centre, Houston, Texas 77030. All equipment shall be brand new, Purchase Order Number will be provided upon award. The contract price shall be firm-fixed. (b) The place of acceptance will be: Michael E DeBakey, VAMC, 2002 Holcombe Blvd, Houston, TX 77030 DELIVERABLES: SEE SOW FOR COMPLETE EVALUATION CRITERIA THE GOVERNMENT DOES NOT ACCEPT RESPONSIBILITY FOR NON-RECEIPT OF QUOTES. IT IS THE CONTRACTOR'S RESPONSIBILITY TO REQUEST AND RECEIVE A CONFIRMATION OF THE QUOTE RECEIPT.
