Page 1 of 6 Statement of Work Department of Veterans Affairs Veterans Health Administration HIG SST Drug Database Subscription March 2018 A. General Information 1.0 Introduction: The Department of Veterans Affairs (DVA), Veterans Health Administration (VHA), requires online access to a commercial, off-the-shelf, drug database (DDB) subscription that will be made available throughout the VHA healthcare system for all healthcare providers, through remote access via OpenAthens accounts, and mobile platforms to include various popular hand-held devices. This database will assist health care providers in their clinical decision making and drug information needs. 2.0 Background: The DVA, which includes 153 medical centers, affiliated Community Based Outpatient Clinics, and other physical work and off-site locations, requires online web access to searchable, current, authoritative, and evidence-based drug information for health care providers. Electronic medical references reduce the need for multiple individual hard copies of expensive print resources and allow for cross searching multiple clinical information resources. This method provides a more efficient manner of retrieving medically necessary information to meet the clinical mission of providing quality care to Veterans within the DVA Network. Health care providers need online tools to be available on VA networked and non-VA networked computers, both onsite and offsite, as well as on mobile devices, to have health information available to provide care for patients. All VA Medical Centers have access to nationally purchased databases and resources that are made available through the VA Library Network Office (LNO). These databases include already subscribed content from: ClinicalKey, OvidMD, the Access Suite from McGraw Hill, and many others. The DDB database required in this statement of work will complement these resources. 3.0 Scope of Work: The contractor shall provide all resources necessary to meet the requirements described in this statement of work for an online, commercial, off-the-shelf, drug database (DDB) subscription. The contractor shall provide 24/7 online web access to the DDB subscription as stated herein for staff at facilities in the DVA (via IP), to their mobile devices (via Apps or mobile websites), or to their remote work areas (via IP or OpenAthens). 4.0 Performance Period: One (1) year from date of award, plus four (4) one (1) year option periods to be exercised at the discretion of the Government. 5.0 Required DDB Subscription Requirements and Salient Characteristics: The contractor shall provide a DDB Subscription which addresses the following requirements and salient characteristics: Provides up-to-date drug information [prescriptions, over-the-counter (OTC), alternative medicine]; Determines dosing, side effects, and interactions; Provides full-text databases on chemical, pharmaceutical, and related biological substances used in clinical patient care; Includes drug reviews, guidelines, clinical reviews, and information from package inserts and drug texts; Contains calculators (dosing, clinical, measurement, laboratory values); Provides access to Black-Box warnings; Provides drug formulary information; Contains a clinical teaching section to assist in communicating information to patients; Provides comparison tables to compare drugs within a class for features like half-life or other valuable information; Comprehensive medication monographs include information regarding Food and Drug Administration (FDA) and non-FDA indications; Contains summaries of clinically relevant primary literature within the resource (more than just a reference to a source); Provides primary literature references and summary data in the resource for comparative efficacy and off-label indications; Contains references to abstracts, tertiary electronic sources and full text articles; Provides a uniform search tool and user interface throughout the product; Updated on a daily basis by a review board of allied health professionals; Provides content on acute care, alternative medicine, drug information, toxicology (including plants, illicit substances and chemicals outside of FDA approved items), teratogenic medications, patient education, and occupational health and safety, including material safety data sheets; Provides the ability to "brand" both web and printed materials with practice site logos and contact information; Provides drug assessments based on manufacturer monographs and peer reviews of primary literature; Provides references to primary literature to support recommendations and/or indications for therapy; Provides side-by-side comparative drug evaluations with primary literature summaries to allow time-effective comparison of more than one agent with one search. Includes concise reviews of trials or case reports including key characteristics of study design (such as including number of patients, p-values, population included); Provides a multitude of web-based calculators designed to meet the needs of all healthcare providers; Provides comprehensive information about drug use in special populations including renal impairment, hepatic impairment, geriatrics, dialysis, obesity and other relevant disease states that will vary by drug; Provides consumer appropriate printable health information on medications, procedures, and disease states. This health information shall be available in at least English and Spanish languages; Provides a drug interaction tool which allows multiple drugs and allergies to be entered for a given patient with output of entry that includes drug-drug, drug-allergy, drug-ethanol, drug-lab, drug-food, drug-tobacco, drug-pregnancy and drug-lactation interactions; Drug Interaction tool will provide severity of interaction; adverse drug reactions will include percentages of patients affected by the interaction, where available, and include case reports; Provides a tool for medication identification based on any information available to the provider (i.e. color, imprint, formulation); Provides administration with tracking tools to include stats on particular components used by the organization; Allows exportability to an iPhone, Blackberry, iPad or Android device for point-of-care decisions; Provides an IV compatibility database that is comprehensive, searchable, and may be drilled to obtain more information or references; Provides pharmacy compounding stability information; Components of the clinical topic review for drug information shall include the following fields: Medical Safety Issues/Alerts. Brand Names. Pharmacologic Category. Dosing (Adults, Elderly, Pediatric) and Forms (capsule, tablet, liquid). Generic Alternative availability. Administration, Use, Contraindications, Warnings/Precautions. Specific Populations (Pregnancy Risk Factor/Lactation/Breast Feeding, Pediatric, Geriatric). Dietary Considerations. Mechanism of Action. Metabolism/Interactions. Pharmacodynamics/Kinetics. References section. Graphics (figures, pictures, graphics, tables) integrated into record/review that can be printed or exported. Hyperlinks to related record/review topics. Provides poison control information including antidote and supportive care recommendations. Includes an intuitive navigation interface for searching, navigating, and reading through database content and the results of a database search; and; Provides patient health information that is printable and can be provided to Veteran patients, their family members, and caregivers; Extensive drug excipient information, on a per NDC basis to cover branded and generic items, including but not limited to dyes, binders, coatings, stabilizers, fillers, antiadherents, flavors, lubricants, preservatives, sweeteners, and vehicles; Specific drug information allowing different salt forms to be compared for clinical differences/indications. A monograph specific to each salt form is required to allow identification of clinical differences between unique salt forms. Separate monographs for biologics and approved BioSimilars to include clinical considerations (including but not limited to dosing, therapeutic uses, interactions, excipients, stability and other clinically relevant differences). 6.0 User Features: The contractor shall provide a DDB Subscription which allows users to: Enter a search phrase consisting of one or more terms or select a term from a drop-down menu of potential matches after typing the first few characters of the term; Enter names of diseases, symptoms, laboratory abnormalities, procedures, drugs, and classes of drugs, to retrieve topical reviews; Enter common synonyms, abbreviations, and acronyms that will be mapped to the appropriate topical review(s) in display results; Search for drug information and retrieve topic reviews containing information about the drug; Search for drug information by retrieving the drug record; Search for drug interactions by entering drug name to display a complete list of interactions for the drug; Choose and use medical specialty calculators interactively by entering required values/units of information to determine mathematical calculation; Sort search results for display using categorization options such as adult, pediatric, geriatric; Find specific text within a topic review using the product s find tool; Select among various output options, including printing all or selected portions of the topical review. 7.0 Access, Content, Support: The contractor shall assure: The product is 508 compliant (http://www.section508.gov/). If product is not 508 compliant, the contractor shall describe measures being taken to bring product into compliance and date, within 12 months of contract award, that product will be 508 compliant. Or, if product is not 508 compliant, the contractor shall describe why no work is being done to make it 508 compliant. The product is compatible Internet Explorer and with most other standard web browsers, which have cookies enabled. To insure privacy, the product uses session cookies rather than persistent cookies. The product is compatible with the Windows environment. All site content and support of the web site is the responsibility of the contractor. This includes tech support to VHA users nationwide. The system shall be accessible 24 hours per day, 7 days a week. System downtime is minimal and VA project manager is notified when such occurs. Information is easily printable. The product s licensing agreement permits authorized VA users free use for educational purposes of all pictures and graphics, and allows sharing of content with other authorized VA users, including emailing among VA colleagues. A single point of contact (and back-up) is assigned to the VA project manager, and available via telephone and email during normal business hours (Eastern time zone; 8:00-5:00). Questions involving website/connectivity issues are responded to within two (2) business days. A toll-free number, email address, and online technical support feature are provided to users. Usage statistics (on an IP and enterprise level) and records of technical service requests are available to the VA project manager, upon request, in a mutually agreed upon format to be determined in the project kick-off meeting. The product is accessible via a variety of personal mobile devices including Apple and Android devices.
