No pass throughs please.) Email Address Phone Number Physical Address Capability statement pertaining to Scope of Work Statement of Work VERA Optimization Professional Services Project James E. Van Zandt VA Medical Center Background Information The James E. Van Zandt VA Medical Center, Altoona, PA has a need to have VERA Optimization Professional Services in order to enhance revenue streams. Scope The vendor will provide 2 four-day weeks of on-site training for the following: Manual VERA audit process VERA provider awareness VERA coder/CDI off-site 2 weeks: data analysis, document and audit prep, final report writing, project mgmt Contractor Requirements Contractor must send training staff that are experienced trainers for this product. Contractor training staff must complete VA Privacy & Information Security training prior to onsite training. The company shall provide customized training and curriculum, a qualified instructor, all training materials, and any other services and/or materials set forth and defined in this Statement of Work. The company shall establish and implement methods of ensuring that all temporary government badges issued to the instructor are not lost, misplaced, misused by unauthorized persons or duplicated for any purpose. Turn-in of any temporary badges to enter government facility shall be returned by company personnel who no longer require access to facility. The service provider shall immediately report the occurrences of lost or duplication of badges to the CO. The company shall not employ persons for work on this contract if such employee is identified to the service provider by the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. Company personnel shall present a neat, professional appearance and be easily recognized as a company employee. The company shall furnish an identifying badge, which shall include: person's name, company name. Each employee shall wear the badge on the outer garments on the front of the body between the neck and the waist so that the badge is visible at all times on government installation. Time Period The company shall perform work required under this contract during the following hours: 0800-1630 hours. Facility & Utilities The government will furnish the facility and utilities at the James E. Van Zandt VAMC, Altoona, PA. The facility includes but is not limited to the classroom, desk, and chairs for all employees being trained. The utilities include electricity, water, sewage, heating, and ventilation for facility. Materials and Aids The company shall ensure that materials and aids are up-to-date. The company shall ensure that materials match the number of students being trained. The company shall ensure that training materials and aids are up-to-date, legible, and available for all students. The company shall ensure that a student critique sheet is available at the end of class in order to evaluate instructors. SELECTION Contract shall be awarded based upon: Past Performance: Satisfaction with provision of training provided at other VA health care facilities will be a selection consideration. Past performance should be with VERA Optimization Professional Services project training and not other unrelated courses with which the contractor may have been involved. Cost: selection will also (in addition to Past Performance) include cost.
