- Page 8 of 8 (i) This is an FBO Notification of solicitation for Cooking Hood Cleaning and Duct Cleaning located throughout the Roseburg VA Medical Center (Roseburg Campus) at the Portland VA Medical Center. A firm-fixed price purchase order is anticipated. Simplified Acquisition Procedures IAW FAR 13 will be used. (ii) The solicitation number is 36C26018Q9521 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Small Businesses and the associated NAICS 561210 code has a small business size standard of $34.5M. The business has been verified for ownership and control pursuant to 38 CFR 74 and is so listed in the Vendor Information Pages database, (https://www.vip.vetbiz.gov); and The Vendor shall be NFPA trained and an authorized technician for the proposed equipment/system such that the OEM warranty and service are provided and maintained. All service associated with the equipment/system shall be performed in a manner that will ensure serviceability and prevent deterioration in value or effectiveness. The vendor shall provide new items ONLY; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. (xvi) For information regarding the solicitation, please contact Jared Zinsmeister at Charles.Hartwickr@VA.Gov Insert (A) STATEMENT OF WORK Full-Service Cleaning Contract Labor and Materials to Clean Cooking Hoods Department of Veterans Affairs Roseburg VA Medical Center 913 N.W. Garden Valley Blvd. Roseburg, OR 97471-6523 1. Background The Roseburg VA Medical Center (RVAMC) requires a full-service maintenance contract to provide all necessary labor and materials to clean cooking hoods located throughout the Roseburg VA Medical Center (Roseburg campus). Cleaning of cooking hoods shall be in accordance with the most current edition of NFPA 17A & NFPA 96, respectively and the particular manufacturer s recommendations. The work shall be performed in a manner that will ensure serviceability and prevent deterioration in value or effectiveness. The contractor shall have only competent and qualified personnel perform the work. This contract includes parts, labor and travel for the maintenance of all equipment covered in this Statement of Work. 2. Scope All work is to be performed on Cooking Hoods located at the Roseburg VA Medical Center. Service contract period of performance is for a base period plus 4 option periods. 3. Specific Tasks Contractor shall be responsible for: Contract shall supply all equipment, materials, and labor to accomplish task Supplying all required safety equipment Service Technician is to provide a service visit report IAW NFPA and Joint Commission documentation requirements. Service visit report shall indicate the following The procedure/activity performed (e.g., inspection, test, or maintenance) The results and date of the activity The required frequency of the activity Inventory of devices, equipment, or other items The organization that performed the activity The name and contact information of the qualified contractor or owner, including lead person for activity NFPA standard(s) referenced for the activity Results of the activity Conducting semi-annual cleaning of the following RVAMC cooking hood systems IAW the current edition NFPA 17A & NFPA 96 standard. Small Hood, Dietetics, Bldg. 71 Cooking Hood, Canteen Bldg. 71 Conducting annual cleaning of the following RVAMC cooking hood systems IAW the current edition NFPA 17A & NFPA 96 standard. Main Hoods, Dietetics, Bldg. 71 Cooking Hood, River House (TCU), Bldg. 81 4. Performance Monitoring The tasks are well defined and can, therefore, be easily monitored by the Government point of contact (POC), who shall represent the government for this process. The contractor performance will be monitored and the work certified by the POC. The POC will assure contractor quality by routine inspections, product sampling, and monitoring of work while work is being performed. 5. Security Requirements All contractor employees are subject to the same level of investigation as VA employees who have access to VA sensitive information or access to VA facilities. The background investigation includes the following requirements: 1) Completed documentation 2) Fingerprints 3) Completion of OPM s e-QIP Questionnaire. The Contractor is required to fulfill all of the security requirements. The Contractor, upon completion of fingerprinting, and an initial suitability determination, may be authorized tentative access to start the performance period of the contract, but only on condition of completion of all security requirements. This requirement is applicable to all subcontractor personnel requiring the same access. The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM) through the VA, the contractor shall reimburse the VA within 30 days. 6. Other Pertinent Information or Special Considerations. The contractor shall protect veterans, visitors, and staff while working at RVAHCS. The contractor shall notify the safety officer if any Cooking Hood systems are not serviced or located within 24 hours. The contractor s service technician shall report to Safety prior to and upon completion of work. The contract shall execute a non-disclosure agreement. The contractor s service technicians will not have any access to view PHI/PPI. 7. Inspection and Acceptance Criteria. The POC is responsible for certifying that the work done under this contract is performed to standard. The POC is also responsible to assure the inspection and acceptance of products provided incidental to services. 8. Place of Performance. Work shall be performed at the following locations. Roseburg VA Medical Center: 913 N.W. Garden Valley Blvd. Roseburg, OR 97471-6523 Remote work for diagnostics and troubleshooting at the contractor s facilities shall be allowed. Remote work shall not incur additional charges. Other locations for the NCO20 catchment area may be added to the contract via bilateral modification after award. This includes OEM trained field service technicians available during the contract to perform emergency repairs or repairs identified during the annual and semi-annual cleaning service calls on an as-needed basis. Repairs will be paid at prevailing per-call rates for travel and labor. 9. Business Hours / Days: Are defined as the time of 7:00AM to 7:00PM, 7 Days a Week, excluding Federal holidays: New Year's Day January 1 * Martin Luther King's Day Third Monday in January President's Day Third Monday in February Memorial Day Last Monday in May Independence Day July 4* Labor Day First Monday in September Columbus Day Second Monday in October Veteran's Day November 11 Thanksgiving Day Fourth Thursday in November Christmas Day December 25* *If the date falls on a Saturday, the Government holiday is the preceding Friday. If the date falls on a Sunday, the Government holiday is the following Monday. In addition to the designated holidays, the Government observes the following days: Any other day designated by Federal Statute Any other day designated by Executive Order Any other day designated by the President's Proclamation Off Business Hours/Days are defined as the time of 7:01PM to 6:59AM, 7 Days a Week, and Federal holidays. 10. Period of Performance. Contract will be one base period plus four option years.
