Description

STATEMENT OF WORK Mindfulness-based yoga therapy services for Veteran Patients. This sources sought notice is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA is currently conducting market research to locate qualified, experienced and interested potential sources. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. NCO 1 Contracting is seeking interested sources for a future solicitation and procurement. At this time, no solicitation exists. Period of Performance: Base plus one option year Requirements And Deliverables: The Contractor shall be required to provide the following deliverables: 1. Year 1: Two (2) 12-week sessions of Mindfulness-based Yoga Therapy within a 12-month period to begin May 1, 2018. Each session will be offered twice a week. The program must incorporate 5 practices: Movement, Breathing, Yoga Nidra, Meditation and Gratitude. Sessions will be provided at: 200 Springs Rd, Bedford MA 01730 Maximum of 20 participants allowed per session Each participant must be provided with a Yoga Practice Guide, Yoga mat, instructional CD/DVD or other materials. Must provide yoga therapy services for a beginner s level participant. Each session will be 75 minutes 2. Year 2: Two (2) 12-week sessions of Mindfulness-based Yoga Therapy within a 12-month period to begin May 1, 2019. Each session will be offered twice a week. The program must incorporate 5 practices: Movement, Breathing, Yoga Nidra, Meditation and Gratitude. Sessions will be provided at 200 Springs Rd, Bedford MA 01730 Maximum of 20 participants allowed per session Each participant must be provided with a Yoga Practice Guide, Yoga mat, instructional CD/DVD or other materials. Must provide yoga therapy services for a beginner s level participant. e graduates of the beginner s level sessions or otherwise qualified for the advanced training as determined by the yoga teacher. Each session will be 75min. 3. Contractor (or designated therapist) must be a certified yoga therapist graduating from 200 hour yoga training program, should have at least 1000 hours and three (3) years of teaching experience since graduating from the 200 hour yoga training program. 4. Contractor (or designated therapist) must have experience working with Veterans patients who may have the following diagnoses: Chronic Pain, Traumatic Brain Injury, Anxiety, Insomnia, Depression, Drug Addictions, or Anger issues. 5. Contractor (or designated therapist) must have experience in working with patients who have Post Traumatic Stress Disorder and must be able to provide proof of such experience. 6. Contractor (or designated therapist) must have experience working with Veterans patients who may have the following diagnoses: Chronic Pain, Traumatic Brain Injury, Anxiety, Insomnia, Depression, Drug Addictions, or Anger-related issues. 7. Contractor (or designated therapist) must have experience in working with patients who have Post Traumatic Stress Disorder and must be able to provide of such experience. 8. Services will be provided at 200 Springs Rd, Bedford MA 01730. Only those services specified herein are authorized. Before performing any service of a non-contract nature, the Contractor shall advise the Contracting Officer of the reason(s) for the additional work, equipment, or service. Changes to the resulting contract shall not be authorized and reimbursement shall not be made for equipment provided or services performed outside the scope of this contract, unless prior authorization is obtained in writing from the Contracting Officer.
