Description

SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 700 South Lincoln Avenue; Lebanon, PA 17042 is conducting a market survey to identify potential sources that are both experienced and qualified to provide for a full service contract that includes inspections, preventive maintenance, and repairs on Hologic equipment, as listed in the Statement of Work (see Schedule A Equipment List). This announcement is issued for informational planning purposes only and is not a request for proposal or for Quote or announcement of a solicitation. Submitting information for this announcement is voluntary and VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this Sources Sought announcement. The North American Industry Classification System Code (NAICS): 56121 Based on this information, please indicate whether your company would be a Small or Large Business, and provide the average amount of sales for the last three (3) years or whether you have a socio-economic designation. Capabilities Statement and Documentation: Sources are asked to provide information to demonstrate their answers to the following items: Company name, address, website, DUNS number, business type, socio-economic status: SDVOSB, Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), general small business, large etc. Provide person of contact (including telephone number & email address). Indicate and explain your capability of providing this service including maintaining any required security clearances and /or additional licensing. Please indicate your company's size under NAICS Code 56121 Please indicate any government contracts held including government agency, contract number, dollar value, period of performance, and government contact information. In addition to a Capability Statement which includes the above information, submissions can include any additional information such as: specifications, brochures, and manuals as attachments, but are not required. Information regarding your firm's capacity and capability to provide such services are to be submitted electronically on or before the date noted on the Announcement.
