Description: Price Schedule Performance Work Statement Contractor Experience Form Past Performance Questionnaire 52.209-7 INFORMATION REGARDING RESPONSIBILITY MATTERS (JULY 2013) Marin County Wage Determination This is a combined synopsis/solicitation for commercial items (services) prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice and in accordance with the simplified procedures authorized in FAR Part 13. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation is being solicited as Total Set-Aside Small Business. The North American Industry Classification System (NAICS) Code 562112, Hazardous Waste Collection. The business Size Standard is 38.5 million, project code is S299 for Other Housekeeping Services. The Government will award a Firm Fixed Price (FFP) contract resulting from this Request for Proposal(RFP) to the responsible offeror whose submission conform to the solicitation. This announcement does not obligate the Government to award a contract nor does it obligate the Government to pay for preparation cost. Project Description: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Drift/Debris Haul Out and Disposal as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract. The offeror must meet the qualification requirement to be eligible for award: 1. Experience Requirement In order to be eligible for award the contractor and or subcontractor performing the work on site shall have at least two (2) years experience on contracts with similar task with a minimum of this size and cost. Complete the Attached Contractors Experience Form 2. Have a Past performance rating of satisfactory or better on current or past contracts within the past 3 years. (End of clause) 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of contract end date. (End of clause) 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 30 (insert the period of time within which the Contracting Officer may exercise the option); provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires.
