This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number 17-235-SOL-00022 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses. Quotes packages must be submitted to the Contracting Officer via email Cordell.Bailey@ihs.gov and in the Subject Line insert, Response to Solicitation 18-235-SOL-00014. (iv) This acquisition is a total small business set aside. (v) There is only one contract line item which is to provide Diabetes SDPI Grant Consultant. services in accordance to the Performance Work Statement (PWS). (vi) The contractor shall provide all services required in the PWS. (vii) The period of performance is one year and will be indicated on the award document. (viii) The attached Clauses and Provisions apply to this acquisition. (ix) The attached Business Associate Agreement will be made part of the contract and the blanks will be filled in by the Contracting Officer with the information of the successful Contractor. (x) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications­ Commercial Items, with its offer. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal.
