- Special Notice Request for Information Only The Department of Veterans Affairs Puget Sound Healthcare System in Seattle, WA is seeking potential sources for microbiology antimicrobial susceptibility testing that meets the requirements below. Respondents should be SD/VOSBs (Service-Disabled Veteran or Veteran Owned Small Businesses) as registered in the VA s VetBiz system (www.vip.vetbiz.gov). This is not a request for competitive quotes; however, any firm that believes it can meet these requirements and is a VIP certified Service Disabled Veteran Owned Small Business (SDVOSB) and/or certified Veteran Owned Small Business (VOSB) vendor may give written notification to the Contracting Officer. System provides windows-based software and customizable data collection fields for clinical trial purposes. System provides ability to enter off-line final isolate identification results at any time during the test procedure (before, during or after the test is complete). System provides clinical epidemiology module to allow customized reports. System saves epidemiology reports so that end user can go back to saved reports and modify only date ranges to generate new report. Epidemiology module provides antibiogram capabilities and duplicate criteria options compliant with M-39 CLSI recommendations. System provides a quality control module to automatically alert the end user to out of range or unexpected results. System tests each analyte for identification tests within the quality control module. System provides susceptibility results for non fastidious gram negative organisms, non fastidious gram positive organisms, streptococcus species, Haemophilus species, yeast isolates, fast and slow growing Mycobacteria isolates, anaerobes System provides true MIC results on standard and custom susceptibility panels. System provides standard MIC plates in accordance with CLSI and FDA recommendations. System can incubate and automatically read up to 64 plates per automated instrument. System can incubate MIC plates offline and utilize automated instant read in the event of overcapacity or power failure. System provides Windows based computer package for data collection and storage. System provides option to read susceptibility panels semi-automatically via image capture. System provides option to read all susceptibility panels manually and record results on windows-based computer platform. System provides automated inoculation of susceptibility panels, frozen or dry format. System provides a shelf life of 12 24 months, room temperature storage, for all identification and susceptibility plates. Advanced Expert Rules-based system that is fully end user customizable. Provides real time digital-image capabilities of susceptibility plates on a windows based software platform. Computer stores digital images of all MIC plates for future review, presentations and/or e-mail Computer provides color-coded interpretations in digital image or manual reads based on CLSI and/or FDA interpretive criteria. Digital image system provides ability to change light options based on organism growth characteristics. System has CLSI and FDA Guideline compliant interpretations in accordance with CLSI Guidelines M100 S22 by June 1, 2013 STATEMENT OF WORK (SOW) Background. Purchase order is requested to order specific products throughout the year necessary to perform microbiology antimicrobial susceptibility testing. Scope. This platform, and associated reagents, is used for bacterial antimicrobial susceptibility testing at the Puget Sound HealthCare System (PSHCS) on veteran s isolated pathogens This contract is for reagents as part of a cost per test agreement. The equipment was acquired during the last five-year contract cycle. Reagents are delivered upon request through the PSHCS Seattle Warehouse. One of several panels that we purchase for this instrument is a custom panel to ensure all antibiotic resistance patterns important in our patient population are identified. These products are mandatory for determining what antibiotics the veteran s provider can use to treat pathogenic organisms. Specific Tasks. This platform, and accompanying reagents, is used for bacterial antimicrobial susceptibility testing at PSHCS on veteran s isolated pathogens. The company also provides reagent kits and yearly preventative maintenance. The vendor will deliver the kits and reagents associated with this contract within a reasonable window of time after ordering has ensued. The vendor will adhere to the priced schedule submitted with the quote of these materials, of which price of shipping is included in the cost of material. The vendor will provide MSD sheets for all media upon request. Times and dates of delivery to be determined by the PSHCS Microbiology Department. Subtask 1 - Integration Management Control Planning. Microbiology employee or supervisor enters request for product from vendor. Purchasing agent places the request with vendor. Vendor ships out product, referencing PO# for payment. Microbiology employee receives product, inspects for discrepancies. Vendor bills for product, Purchasing Agent checks for delivery completeness from end-user. Purchasing agent completes receiving protocol in computer. Microbiology Department uses product to perform antimicrobial susceptibility testing for veteran patients. Task 2. Subtask 2 - Contract Management. Products are ordered at Seattle by the Microbiology Department in coordination with Administrative Specialist. The order is sent to the vendor via email, referencing the correct purchase order given from Fiscal and Contracting. Any delivery delays are sent via email to the purchaser in the Microbiology Department, who coordinates with vendor. Vendor sends invoices for payment to the VA Accounting Department in Austin, TX. Austin Automated Payment System posts the invoices for the laboratory to review, and then the invoices will be certified after confirmation of received items. Performance Monitoring The Microbiology Department must adhere to specific standards for all laboratory consumables. These standards are applied to the products ordered from this vendor. COTR and/or any Microbiology employee are responsible to monitor all products for quality. The microbiology laboratory QA Technologist and Supervisor will monitor the system for minimum performance. In the event performance is compromised by either reagent or instrument failures, the company will be contacted for remediation. Security Requirements There are no security related components to this contract request. The vendor will not have access to protected patient information. Protected patient information will not be input into the vendor computer and the vendor computer associated with the testing instrumentation will not be interfaced with any VA networks Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There is no government furnished equipment associated with this contract. Other Pertinent Information or Special Considerations. Identification of Possible Follow-on Work. None Identification of Potential Conflicts of Interest (COI). None Identification of Non-Disclosure Requirements. Contractor will not have access to any veteran names or diagnosis. Packaging, Packing and Shipping Instructions. All reagents shall be shipped according to IATA shipping regulations and in accordance with instruction provided by the Microbiology Laboratory at PSHCS. Inspection and Acceptance Criteria. Microbiology POC, Supervisor and any other Microbiology employees have specific lab standards to maintain for all products under their care. Upon inspection all reagents shall be shipped according to IATA shipping regulations and in accordance with instruction provided by the Microbiology Laboratory at PSHCS. All shipments will be received at the appropriate temperature (storage conditions) commensurate with instrument and reagent standards. Risk Control Microbiology is responsible and will accept any risks associated with product, notified by vendor. Only quality controlled and verified results will be released to the medical record. Place of Performance. Specific area to receive and work with product from vendor is the Microbiology lab at the Seattle Division of VA Puget Sound Medical Center. Period of Performance. There is no defined period of performance over and above the warranties supplied by the company (vendor). Delivery Schedule. Items are to be delivered as requested by the PSHCS Microbiology Laboratory
