- Price schedule for all IT supplies and equipment needed. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. The Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF) is requesting for offers for a contractor to provide various IT Supplies and Equipment for several departments. The contractor shall be registered in the System for Award Management (SAM) system, and maintain an Active Entity account throughout the contract term. This synopsis/solicitation is Small Business Set Aside only. EVALUATION CRITERIA CCHCF reserves the right to issue a single award or multiple contracts to any contractor(s) whose quote represents the best value as define by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. To be considered for award, your offer shall include the following: 1.Provide Price Listing of IT Supplies and Equipment. 2.Provide evidence of Past Performance and Experience working with government facilities. 3.Provide your DUNS and Bradstreet Number. Due to urgency no questions may be submitted. RFQ Closing Date July 18, 2018 @ 2:00 pm MST.
