- Wage Determination Dated 06 01 2018 SBU and Fitness Determination Policy Key Sustainable Products CLIN 009 Specs CLIN 007 Specs CLIN 005 Specs CLIN 003 Specs CLIN 001 Specs Terms and Conditions 1442 and 1442a Reps and Certs Solicitation The General Services Administration (GSA) is setting aside the contract under the following clause found in the Agreement: 52.219-6, Notice of Total Small Business Set Aside SUBJECT TO MULTIPLE AWARD: GSA will be accepting price quotes for five locations in Anchorage with the intent to award one to five contract(s). Offerors are asked to provide itemized pricing for each CLIN group (example: CLIN 0001 and CLIN 0002; CLIN 0003 and CLIN 0004, and so forth) Quotes shall be on the attached 1442 Continuation sheet and you are required to submitt CSI Masterformat Level 3 Break-Out to support pricing (to include all sub-contractor quotes). Demo and replace concrete for various projects in Anchorage Alaska. DESCRIPTION OF WORK Contractor shall supply all supervision, equipment, labor and materials necessary to: 10PCB-1-18-0056, Demo and replace approximately 250 SF of 12” wide x 6” deep concrete landscaping border along the 4th Ave and F Street sidewalks at the Anchorage Historic Federal Building (HFB). 10PCB-1-18-0057, Demo and replace approximately 300 square feet (SF) of failing concrete driveway pad along the 3rdh Ave and F Street vehicle entrance at the Anchorage Historic Federal Building (HFB). The existing pad is deteriorating with exposed rebar in several sections. 10PCB-1-18-0062, Demo and replace approximately 480 square feet (SF) of broken and worn 4-inch thick concrete walkway along 7th Ave at the Anchorage Federal Building (AFB). Additionally, nearby ADA curb ramp and the concrete immediately adjacent to it have heaved which make the ramp extremely difficult to use. GSA requires action to demo and replace ramp and walkway concrete to remove obstacles. Reference attached A-1 Layout. See specific requirements listed in Statement of Work and Attachments 10PCB-1-18-0063, Demo and replace approximately 180 lineal feet (LF) of worn 6- inch concrete curb adjacent to building walkway at the Anchorage Federal Building (AFB) G Mod Annex. Existing curb ramp to remain in place. Reference attached A-1 Layout. The existing pad is deteriorating with exposed rebar in several sections. 10PCB-1-18-0065, Demo and replace approximately 260 SF of exposed aggregate concrete walkway at the Anchorage Historic Federal Building (HFB).
