- STATEMENT OF WORK Roseburg VA Health Care System Inspection and cleaning of Cooking Hood Extinguishing Systems Title of Project: Inspection and cleaning of Cooking Hoods and Cooking Hood Extinguishing Systems. Scope of Work: Provide all necessary labor and materials to clean, inspect, service and maintain the cooking hood extinguishing systems and cooking hoods located throughout the Roseburg VA Medical Center (Roseburg campus). Inspections shall be in accordance with the most current edition of NFPA 17A & NFPA 96, respectively and the particular manufacturer s recommendations. The work shall be performed in a manner that will ensure serviceability and prevent deterioration in value or effectiveness. The contractor shall have only competent and qualified personnel perform the work. Contractor shall be responsible for: Supplying all equipment, materials, and labor to accomplish task Supplying all required safety equipment Protecting veterans, visitors, and staff while working at RVAHCS. Other requirements are listed in VAF 2237 for this contract. Notifying the safety officer if any Cooking Hood systems are not serviced or located within 24 hours. Service Technician is to report to Safety prior to and upon completion of work. Service Technician is to provide a service visit report IAW NFPA and Joint Commission documentation requirements. Service visit report shall indicate the following The procedure/activity performed (e.g., inspection, test, or maintenance) The results and date of the activity The required frequency of the activity Inventory of devices, equipment, or other items The organization that performed the activity The name and contact information of the qualified contractor or owner, including lead person for activity NFPA standard(s) referenced for the activity Results of the activity Conducting semi-annual fire suppression system service of the following RVAMC cooking hood fire suppression systems IAW the current edition NFPA 17A & NFPA 96 standard. Gaylord Quencher Water Wash System, Dietetics, Bldg. 71 (2) Gaylord Quencher BDL Non-Water Wash System, Dietetics, Bldg. 71 Gaylord Quencher Water Wash System, Canteen Bldg. 71 Ansul Wet Chemical System, Canteen Bldg. 71 VA to provide: Site plan of building layout Restroom facilities Contractor ID badges Type of Contract: This will be a base and option year contract
