- Page | 1 of 4 Statement of Work 1. Description of Services: The Contractor shall provide all management, licenses, tools, supplies, equipment, transportation and labor to provide inspection, testing and maintenance of the Electrical Power Distribution System at the James A. Lovell Federal Health Care Center (FHCC) located in North Chicago, IL. 2. Scope of Work: The contractor shall come on site to FHCC and Clean, Tighten, Lubricate, Inspect, Calibrate and Test the electrical distributition system at the medial health care center. All services provided under this contract must be performed in conformance with the Occupational Safety and Health Administration (OSHA) and the Natural Electrical Code. Specific Tasks: Clean- All the contaminants that are not compatible with electricity. These contaminats include dust, dirt, moisture, water, rust or naything else that could reduce the efficiency of the electricial system. Tighten- Loose electrical connections are heat sources that reduce the efficiency of the electric system and ultimately result in the destruction of a portion or portions of the system. An energized alternating-current system is subject to constant vibration, and therefore, connections frequently become loose. Lubricate- Direct-Acting, circuit breakers, relay-atuated power circuits breakers and switches contain many pivoting or sliding mechanical parts. These parts require lubrication and exercising so that they do not become inoperable because of nonuse. Inspect- The ratings and settings of all electrical devices are checked to assure that they are in accordance with good design and load changes. Tested & Calibrated- The system or protective devices are subjected to abnormal electrical conditions; the operation of the system or devices is compared to the maufacturers specifcations and plant design requirements. All inspections, calibrations, repairs and tests were in accordance with the following codes: National electrical code-NEC National Electricial Manufacturers Association-NEMA American Society for Testing and Materials-ASTM Institute of Electrical and Electronic Engineers-IEEE National Electrical testing Association- NETA American National Standards Institute-ANSI The Contractor shall install equipment in accordance with the manufacturers installation instructions. The Contractor shall report deficiencies that are deemed critical or catastrophic immediately to the Contracting Officer and Contracting Officer s Representative (COR) for immediate actions. Deliverables: f) Reports- A complete written report of the work performed shall be provided to FHCC within seven (7) calendar days of testing completion. Information shown in the report shall be as following: 1) Company s name, addresses, telephone, & FAX numbers. 2) Name and signature of contractors who perform the maintenance and testing. 3) FHCC Work Contract Number, name and number of VA Contracting Officer. 4) Date and Time of work. 5) Copies of contractors valid licenses, professional and training certificates. 6) Descriptions & model number of specialized tools/equipment used (i.e. torque wrench or infra-red scanning camera). 7) Location, Type, Name, and nameplate information of electrical equipment to be maintained and tested. 8) Descriptions of work items. 9) Test data. 10) Reference materials such as equipment manufacturer s specifications, coordination study, etc. 11) Remarks on conditions of electrical equipment. List all deficiencies, if any. 12) Recommended corrective actions, if any. Submit two (2) hard copies of the complete written report, and one (1) CD-ROM or DVD of the electronic version of the report in Microsoft Word format to the Chief of Facilities Management/Engineering Service, within seven (7) calendar days of testing completion. All reference materials shall be included in the electronic version, either through scanning or other means of electronic text import methods. g) Reports- A complete written report of the work performed shall be provided to FHCC within seven (7) calendar days of testing completion. Description Of Equipment Requiring Inspection, Testing and Maintenance at FHCC: Thirty Five (35) Medium Voltage (5KV) Citcuit Breakers Visual and mechanical inspection Clean and Lubricate Verify primary and secondary contact wipe. Check Cell fit and element alignment. Check proper operation of cubicle shutters. Measure contact resistence. Perform insulation resistence and Vacuum Integrity with applicable. Fuctional trip test of breaker by protective relays. Perform Infrared inspections. One Hundred And Three (103) Protective Relays Inspect relays Perform all settings in accordance with set points. Check alignment, connections, and tap plug Perform pickup pn each operations element. Perform timing test at 3 points on the time dial curve when applicable. Compare settings to coordination study where available. Medium Voltage Switches (23) and Pedestals (17), Junction Boxes Visual and mechanical inspection. Lubricate and clean the switch. Perform doctor (contact resistence) and insulation tests on switches when applicaple. Perform oil analysis of pedestal dielectric fluid, when applicable. Check bus and cable connections for proper torque. Perform infra-red inspection. Low Voltage Power Circuit Breakers and MCCBS (261) Visual and mechanical inspections. Check case for cracks and other defects. Lubricate and clean the breaker. Perform trip time tests on breaker time current devices where applicable. Exercise and operate the breaker/switch to ensure proper operation. Perform doctor and insulation tests. Perform trip time tests on breaker time current devices. Exercise and operate the breaker to ensure proper operation. Perform infra-red inspection. Switchboards, Panel Boards (30), Relays (105), Breakers, ETC Visual and mechanical inspection. Check case for cracks and other defects. Lubricate and clean the breaker/switch. Perform trip time tests on protective relay devices. Perform dustor and insulation tests where applicable. Exercise and operate the breaker/switch to ensure proper operation. Inspect bus insulators an cable connections for tightness. Perform infra-red inspection. 4. Contractor Requirements: Contractor shall perform work in a professional manner and to the highest standards. Contractor shall check in with the electric shop at time of arrival and checkout as defined in service hours. Contractor must be licensed/certified by the State of Illinois to perform all required work specified in the Statement of Work. Contractor shall be required to follow all Infection Control Risk Assessment (ICRA) requirements set forth by the infection control staff. Contractor shall remove all trash and debris at the completion of job. The work area is to be kept clean at all times. The Contractor is to insure that all tools, equipment and material are kept and placed in an orderly fashion to have as little impact on daily operations of the hospital, staff and patients. Owner s manual will be supplied as part of the close out documentation. Contractor shall be required to complete a Captain James A. Lovell Federal Health Care Center Service Ticket when the work is completed. Contractor shall perform a function test in the presence of a VA employee designated by the electric shop supervisor. Contractor shall submit a proposed schedule with their quote. The schedule shall be in the form of a progress chart of suitable scale to indicate the work scheduled for completion by any given date during the work period. The proposed schedule shall be reviewed by the Lovell FHCC Facilities Management/Engineering Service; any requirements for adjusting the schedule (e.g. to minimize disruption to the FHCC) shall be incorporated by the Contractor. 5.
