- Description
- The Syracuse VA Medical Center has a need for Community Based Outpatient Clinic (CBOC) services to provide primary care services to Veteran patients in a private hospital, office or clinic environment. The CBOC must be located in Cayuga County in the State of New York. Request for Proposal (RFP) 36C24218R0013 will be available for download from this website on or about July 16, 2018. No hardcopies will be sent. Questions must be submitted in writing via email to the Contract Specialist at Lisa.tompkins@va.gov. Questions must be received a minimum of ten (10) business days prior to the closing date listed in the RFP. If warranted, responses to inquiries will be incorporated into a written amendment posted to the Federal Business Opportunities (FBO) website at http://www.fbo.gov. Verbal questions and phone calls will not be addressed. Offerors are encouraged and are responsible for routinely checking this website for any solicitation amendments that may be issued throughout the proposal period. This is an unrestricted procurement. A single award contract will be made. Contractors are required to be registered in System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM before contract award. The NAICs code for this requirement is 621498 All other Outpatient Care Centers and the Size standard is $20.5 Million. The contract will cover a base year and four (4) one-year option periods to be exercised at the Government's discretion. Contractor shall provide medical facilities, health care providers, medical equipment, supplies and administrative support. Request for Proposal (36C24218R0013) will be available for download from this website on or about July 16, 2018. No hardcopies will be sent out. Questions must be submitted in writing to the contracting officer. Contractors are strongly encouraged to do so within ten (10) calendar days after issuance of the Request for Proposal (RFP).
