Attachment 2 SCLS Index of Wage Determination for the State of Kentucky Attachment B WRP TSI Plans for the Coalfield and Eastern Kentucky Attachment A Tool for Assessment and Treatment of Reforested Bottomland Hardwood Stands on Wetland Reserve Easements Attachment 1 Statement of Work (SOW) Timber Stand Improvement (TSI) Plan Standard Form (SF) 1449 121A2318Q0028 (pages 1 and 2) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number 121A2318Q0028 is being issued as a Request for Quotation (RFQ) and is being conducted as a 100% Total Small Business Set-Aside. The Small Business size standard is based on the North American Industry Classification System (NAICS) code 115310 Support Activities for Forestry with a size standard of $7.5 million average annual income. This requirement will be awarded as a commercial item, fixed price purchase order under the procedures at FAR Part 12. PROJECT DESCRIPTION. To assist NRCS with Timber Stand Improvement Plans on acreage that received vegetative restoration (tree planting through the NRCS Wetland Reserve Program (WRP). See Statement of Work, Exhibit A for more details. PROJECT DURATION. Contract award through December 30, 2018. First six (6) plans must be submitted within the first 30 days, than 6 plans per month until all 28 plans are completed. SUMMARY Contractor shall furnish provide and furnish all supervision, labor, materials, tools, equipment, transportation, supplies and travel necessary to provide the required services to assist NRCS with Timber Stand Improvement Plans on WRP easements throughout the State of Kentucky in accordance with NRCS specifications, and terms and conditions of the contract, and other attachments provided. NRCS will provide contractor access to easements along with plan/topo map and location map for each individual easement. TELECONFERENCE. A Teleconference will be held for this project; the purpose of this conference will be to answer questions regarding, or requests for clarification of, the solicitation documents. Statements or representations made during the teleconference are not legally binding. Changes resulting from the teleconference are official only if issued through an Amendment to the Solicitation. (a) Participants will meet at: USDA NRCS Madisonville Service Center 1095 National Mine Dr, Ste. 4 Madisonville, Kentucky 42431 DATE: July 23, 2018, TIME: 10:00 am (Central Time) Contracting Officer Representative (COR) Ray Toor USDA NRCS 1095 National Mine Dr, Ste 4 Madisonville, KY 42431 Telephone: (270) 825-2414 PREPARATION/SUBMISSION OF OFFERS Addendum to 52.212-1, Modify (b): Submission of Offers to read as follows: Quotes shall be submitted on COMPANY LETTERHEAD, SIGNED, DATED, to include the Standard Form (SF) 1449 (page 1 and 2) and Pricing Schedule (page 3), and sent via email to Danny Mandell, Contracting Officer, email: Danny.Mandell@wdc.usda.gov, no later than August 2, 2018, 4:00 PM Central Daylight Time (CDT). The offeror shall provide the NRCS with a Firm Fixed Price quote that is in accordance with this solicitation. See PRICING Schedule (page 3) Potential Quoters are reminded to submit all the required documents, stated above, by the due date, in addition, provide copy of their active SAM registration to be considered for award of a federal contract. Failure to provide the above requirements will make their offer non-responsive. Potential Quoters are advised that they are responsible for obtaining and acknowledging any and all amendments to the solicitation. It is the Quoters responsibility to view the FedBizOpps website at www.fbo.gov, daily as no other notification of amendments will be provided on any revisions to this announcement or other notices. SYSTEM FOR AWARD (SAM) REGISTRATION: All prospective contractors must register in the System for Award Management (SAM) registration database at www.sam.gov, to be considered for an award of a federal contract.
