This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ) and the solicitation number is 36C24418Q0444. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 effective January 24, 2018. The associated North American Industrial Classification System (NAICS) code for this procurement is 238290, Other Building Equipment Contractors with a small business size standard of $15.0 Million. The Department of Veterans Affairs, Wilkes-Barre VA Medical Center, 1111 East End Boulevard, Wilkes-Barre, PA 18711-0026 is seeking to purchase Install Ceiling Lift Service. All interested companies shall provide quotations for the following: The anticipated Period of Performance will be 60 days following Contract award and includes the following Contract Line Item Numbers (CLIN): CLIN 0001: Install patient ceiling lifts for 2nd Floor Community Living Center. 1 Bed and 2 Lifts in each room (60 days following Award). CLIN 0002: Install patient ceiling lifts for Intensive Care Unit. 1 Lift and 1 Bed in each room (60 days following Award). CLIN 0003: Install patient ceiling lifts for 4 East Inpatient. 1 Lift and 1 Bed in each room (60 days following Award). CLIN 0004: Install patient ceiling lifts for Short Procedure Unit. 1 Lift and 1 Bed in each room (60 days following Award). CLIN 0005: Supply Ancillary Equipment in accordance with the SOW (60 days following Award). CLIN 0006: Upgrade Motor Capacity of existing lift in Community Living Center (CLC) Patient Room 3, from 130 to 1,014 pounds (60 days following Award). See also Attachment 2, Price Cost Schedule. The Schedule of Supplies and Services is included as Attachment 2 to this solicitation. Service Contract Act Wage Determination Number 2015-4239, Revision Number 5, dated 01-10-2018, applies to this acquisition and will be incorporated into the resulting contract. A copy of the Wage Determination is included with this solicitation as Attachment 3. Justification and Approval documentation (redacted) for Brand Only (Liko [Hil-Rom] ceiling lifts) is included as Attachment 4. Technical Specifications are included as Attachment 5. Any award resulting from this solicitation will be issued on a Standard Form 1449. NOTE: To receive an award resulting from this solicitation, offerors MUST be registered in the System for Award Management (SAM) database IAW FAR 52.212-(k).
