- Attachment F - Past Performance Chart Attachment E - Sample Trash Report Attachment D - Wage Determinations Attachment C - Price Schedule for sites added after award Attachment B - Facility Information and Pricing Schedule Attachment A - Statement of Work Solicitation 4BW-18-A-0018 The United States Postal Service, Facilities Services CMC, 3300 S Parker Rd., Aurora, CO 80014, UNITED STATES The Facilities Services CMC of the United States Postal Service is soliciting proposals for Trash Removal Services for the USPS Southern Area. This is a combined synopsis/solicitation for commercial services. One or more awards will be made from this solicitation. The award will be made as a Firm-Fixed Price with Incentive contract. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this effort is 4BW-18-A-0018. This requirement is issued as a Request for Proposal (RFP). The North American Classification System (NAICS) Code is 562111, Trash collection services. The period of performance will be a base period of 24 months and four 24-month options. A best value tradeoff analysis process will be conducted. Proposals will be evaluated under two primary factors: 1) Technical Proposal and 2) Business Proposal. Price is more important than technical factors. The final selection will be made using the qualitative factors and price as the tradeoff factors for offers ultimately determined to be technically acceptable for all factors evaluated. An offeror rated overall as unacceptable (including unacceptable but capable of being made acceptable) shall not be eligible for award. The following evaluation factors will be used to evaluate offers: 1. TECHNICAL PROPOSAL: The technical proposal shall not contain reference to cost; however, resources information, such as data concerning labor hours and categories, materials, subcontracts, if required, shall be contained in the technical proposal so that your understanding of the SOW may be evaluated. It must disclose your technical approach in as much detail as possible, including, but not limited to, the requirements of the technical proposal instructions. 2. BUSINESS PROPOSAL: The Offeror shall include all of the below listed information in this part. Do not return the entire solicitation - we already have it - rather just complete and return the pages identified below. This information is mandatory. Failure to comply may result in the Proposal being deemed non-responsive and disqualified. - Complete Facility Information and Pricing Schedule at Attachment B. All areas and option years must be completed in order to be considered for award. In addition, include acknowledgement of any amendments issued, if any; - Complete DUNS Number Request - Complete Provision 4-3, Representations and Certifications - Complete and return W-9 - VETS 4212 registration confirmation Offerors are advised that the following Provisions and Clauses are applicable to this procurement: Provision 4-2 Representations and Certifications - All offerors must include a completed copy of the provision 4-2 with their offer Clause 4-1 General Terms and Conditions Clause 4-2 Contract Terms and Conditions Required to Implement Policies, Statutes or Executive Orders All attachments associated with this requirement are provided as stated below: Solicitation 4BW-18-A-0018 Attachment A - Statement of Work Attachment B - Facility Information and Pricing Schedule Attachment C - Price Schedule for sites added after award Attachment D - Wage Determination Attachment E - Sample Trash Report Attachment F - Past Performance Chart Proposals are due by July 30, 2018 at 4:00 pm MT via email to YW5FB0@usps.gov.
