- ATTACHMENT 6 - AID1420-17 Employee Biodata Sheet ATTACHMENT 5 - Past Performance Forms and CVs of Key Personnel Solicitation for Construction Oversight Services for 3 Intermediary Warehouses in Zambezia, Mozambique Request for Proposal (RFP) No. 72065618R00018 - Design Review and Construction Oversight Services for Three Intermediary Warehouses in Zambezia Province Dear Offeror, The United States Government, represented by the U.S. Agency for International Development (USAID) Mission in Mozambique, is seeking proposals to provide and perform professional architectural and engineering (A&E) technical services. This contract will support design review and construction oversight "Fiscalização de Obras" for the design, construction and other related services for intermediary warehouses. Additional details are further described in the attached Request for Proposals (Solicitation/RFP), for the following pharmaceutical warehouses: • Three Intermediary Warehouses in Zambezia Province o Erego-Ile o Mopeia o Mocuba USAID/Mozambique anticipates awarding a single Firm-Fixed Price type contract as a result of this solicitation. The estimated cost for this procurement is approximately $600,000.00. Implementation of this activity, for an estimated period of 34 months with no option years, is subject to availability of funds. The Offerors will provide the lowest possible price corresponding with the technical proposal. The North American Industry Classification System (NAICS) code for this acquisition is 541310. Offerors shall refer to Section L (Instructions for the Proposal Preparation and Submission) of the solicitation for additional information regarding proposal requirements. It is important that Offerors submit proposals on time. Proposals received after the submission due date will not be reviewed or evaluated subject to the Contracting Officer's discretion. Section C (Scope of Work) of this solicitation provides details on the activities required to be implemented. Section M (Evaluation Criteria) states the criteria by which proposals will be evaluated. Questions for this solicitation can be submitted to USAID/Mozambique no later than July 16, 2018 at 03:00 pm Maputo local time, by e-mail to Faizallet Sultan at fsultan@usaid.gov with a copy to eluma@usaid.gov. The issuance of this solicitation in no way obligates USAID to award a contract, nor does it commit USAID to pay for any cost incurred in the preparation and submission of a proposal. Furthermore, the US Government reserves the right to reject any offer, if such action is considered to be in the best interest of the Government. Please submit your proposal no later than the August 10, 2018 at 3:00 pm Maputo local time. Please note that early submission is encouraged and welcomed.
